肿瘤患者外周血T淋巴细胞亚群及NK细胞比例检测的临床意义

摘　　要：目的探讨肿瘤患者外周血淋巴细胞亚群和NK细胞表达水平检测的临床意义。方法选取621例肿瘤患者(包括肺癌、肝癌、乳腺癌、胃癌和鼻咽癌)为肿瘤组,同期115名健康人为健康组,采用流式细胞术检测外周血中T淋巴细胞亚群、CD19^+B细胞和NK细胞的细胞免疫表达水平并进行对比分析。结果肿瘤组外周血的CD3^+、CD4^+、CD19^+细胞比例及CD4^+/CD8^+比值均低于健康组,NK细胞比例高于健康组,差异均有统计学意义(P<0.05)。亚组分析结果显示,不同肿瘤间的CD8^+细胞比例差异无统计学意义(P>0.05),CD3^+、CD4^+、CD19^+、NK细胞比例及CD4^+/CD8^+比值差异均有统计学意义(P<0.05)。结论肿瘤患者外周血中不同淋巴细胞亚群及NK细胞的变化对早期发现和早期诊断具有指导意义,同时也可作为临床监测肿瘤患者细胞免疫功能状态的指标之一。Objective To investigate the change of peripheral lymphocyte subsets and NK cells in tumor patients and its clinical significance.Methods 621 patients with tumor(including lung cancers,liver cancers,breast cancers,stomach cancers and nasopharyngeal cancers)were selected as the tumor group and 115 healthy people as the healthy controls in the same period.The flow cytometry was adopted to detect the expression levels of peripheral blood T lymphocyte subpopulations,CD19^+ cells and NK cells.Results The proportion of CD3^+,CD4^+,CD19^+ cells and the ratio of CD4^+/CD8^+ in peripheral blood of the Tumor group were lower than those in the healthy controls(P<0.05).The proportion of NK cells was higher than the healthy controls(P<0.05),they are significantly different.The result of subgroup analysis showed that,there was no difference in the proportion of CD8^+ between different tumors.The proportions of CD3^+,CD4^+,CD19^+,NK cells and the ratio of CD4^+/CD8^+ were different(P<0.05).Conclusion The changes of different lymphocyte subsets and NK cells in peripheral blood of patients with tumors have guiding significance for the early diagnosis,and can also be used as one of the indicators for the clinical monitoring of cellular immune function in tumor patients.
