结直肠癌患者血清lncRNA H19表达水平与术后化疗效果的相关性分析

摘　　要：【目的】探讨结直肠癌患者血清长链非编码RNA(lncRNA)H19表达水平与其预后的相关性。【方法】选取2013年1月至2015年12月于本院行根治术+辅助化疗的86例结直肠癌患者为研究对象(观察组),选取同期于本院健康体检的90例志愿者对照组。采用qRT-PCR检测两组血清lncRNA H19表达水平,分析观察组患者血清lncRNA H19表达水平与化疗后效果的相关性。【结果】qRT-PCR结果显示,观察组术前血清lncRNA H19水平为(1.63±0.35)显著高于对照组(1.00±0.11),差异具有统计学意义(t=16.259,P<0.01)。以观察组血清lncRNA H19表达水平的中位数2.03为临界值.将患者分为高表达组和低表达组,两组各43例。高表达组肿瘤最大径、低分化、淋巴结转移、侵及浆膜、Ⅲ-Ⅳ期的比例、术前CEA水平显著高于低表达组,差异有统计学意义(P<0.05)。高表达组和低表达组化疗后胃肠道不适、骨髓抑制、肝肾功能损伤、神经毒性、黏膜炎发生率比较,差异无统计学意义(P>0.05)。高表达组患者中位总生存时间(OS)、无进展生存期(PFS)分别为33.6个月、22.9个月显著低于低表达组的55.8个月、43.3个月,差异均有统计学意义(P<0.05)。【结论】结直肠癌患者的血清lncRNAH19水平高表达者的生存状况较差,术后化疗的敏感性更低.但与化疗耐受性未见明显相关性。【Objective】To investigate the correlation between,the expression of serum long-chain non-coding RNA(IncRNA)H19 and prognosis in patients with colorectal cancer.【Methods】Eighty-six patients with colorectal cancer who underwent radical operation and adjuvant chemotherapy in our hospital from January 2013 to December 2015 were selected as the study subjects(observation group).90 volunteers who underwent physical examination in our hospital during the same period were selected as the control group.The expression level of lncRNA H19 in serum of the two groups was detected by qRT-PCR,and the correlation between the expression level of IncRNA H19 in serum of the observation group and the effect of chemotherapy was analyzed.【Results】The results of qRT-PCR showed that the level of serum lncRNA H19 in the observation group was(1.63±0.35)significantly higher than that in the control group(1.00±0.11),and the difference was statistically significant(t=16.259,P<0.01).The critical value 2.03 was set as the median level of serum lncRNA H19 expression in the observation group,and the patients were divided into two groups:high expression group and low expression group,43 cases in each group.The maximum diameter,low differentiation,lymph node metastasis,invasion of serosa,the propor-tion of stageⅢ-Ⅳand preoperative CEA level in the high expression group were significantly higher than those in the low expression group(P<0.05).There was no significant difference in the incidence of gastrointestinal discomfort,bone marrow sup-pression,liver and kidney dysfunction,neurotoxicity and mucositis between the high and low expression groups after chemotherapy(P>0.05).The median total survival time(OS)and pro-gression-free survival(PFS)in the high-expression group were 33.6 months and 22.9 months respectively,which were significantly lower than those in the low-expression group(55.8 months and 43.3 months),with statistical significance(P<0.05).【Conclusion】The survival of patients with high expression of serum IncRNA H19 i
