肝癌切除术中Glisson蒂横断与第一肝门阻断的应用比较

摘　　要：目的比较肝癌切除术中Glisson蒂横断与第一肝门阻断的应用。方法为展开此次研究将2017年1月至2018年5月进入本院进行治疗的肝癌患者56例作为研究基础,将参与研究患者分为均数相等的两个小组,两组参与研究患者例数相同,均为28例,分组采用随机信封方式,在尊重患者护理意愿基础上将其分为两组,对照组采用第一肝门阻断手术治疗方法,观察组应用Glisson蒂横断,对比两组手术质量以及术后的各项恢复指标。结果观察组患者经过Glisson蒂横断后,肝脏血流阻断时间与传统第一肝门阻断方法比较差异无统计学意义,ALB对比方面,差异无统计学意义,并发症发生概率方面对比,差异无统计学意义;但是手术时间、术中出血量与术后3 d引流量、肝功能各项指标对比,观察组均比对照组更加具有优势(P<0.05)。结论肝癌切除术中Glisson蒂横断与第一肝门阻断均为经常使用的治疗方法,两者对比而言Glisson蒂横断治疗方法更加理想,手术效果优势明显,患者在术后的恢复效果更好,临床推广价值较高。Objective To compare the application of Glisson pedicle transection and first hepatic hilus occlusion in hepatectomy.Methods In order to carry out the study,56 patients with liver cancer who entered our hospital for treatment from January 2017 to May 2018 were divided into two equal groups with the same number of patients.All 28 cases were divided into two groups:the control group was treated with the first hepatic hilum occlusion operation,the observation group was treated with Glisson pedicle transection,and the quality of operation was compared between the two groups.Item recovery index.Results After Glisson pedicle transection,hepatic blood flow occlusion time was compared with the traditional first hepatic portal occlusion method in the observation group.There was no significant difference in ALB,no statistical significance in ALB,and no statistical significance in the probability of complications.However,the time of operation,the amount of intraoperative bleeding,the drainage volume of blood and liver function in the observation group were significantly higher than those in the control group(P<0.05).Conclusion Both Glisson pedicle transection and first hepatic hilus occlusion are frequently used in hepatectomy.Compared with Glisson pedicle transection,Glisson pedicle transection is more effective,and the recovery of patients after operation is better.The value of clinical popularization is high.
