右位胃内镜特征分析

摘　　要：目的总结左侧卧位状态下右位胃的胃镜下特征。方法回顾性分析我院9例共12次右位胃胃镜资料,找出其与正常胃的不同之处,并记录这些不同之处的出现率。结果通过贲门,9例患者的胃底均位于右侧,其中6例较为明显;所有患者的胃腔走行均为左侧,但比较典型与正常走行呈完全相反者6例;9例患者胃底均未见黏液湖,8例胃窦出现明显黏液湖,1例黏液湖不明显;低位反转观察,9例患者中5例胃角位于后上方,3例位于正上方,另1例正常;9例患者中8例十二指肠降部入口位于球部左下方,1例位于右侧。结论胃窦有黏液湖而胃底无黏液湖,十二指肠降部入口位于球部左下方为右位胃胃镜下重要且稳定的特征,胃底位于贲门右侧,胃体左侧走行是相对特异但可能因为不典型而容易被忽视的特征。Objective To summarize the characteristics of dextrogastria by gastroscopy in the left lateral decubitus position.Methods The gastroscopy results of 9 dextrogastria patients were analyzed retrospectively to find the differences compared with the normal.The frequency of the differences among these patients was recorded.Results The gastric fundus was found on the right of the cardia in all these patients,but 6 patients were not obvious.Although all the gastral cavity run to the left side,but only 6 run contrary to the normal direction completely.The gastric angles were on the upper right of the view in 5 patients.There were lakes of mucin in the gastric antrum in 8 patients while no lakes of mucin were found in gastric fundus in all the patients.In 8 patients,the duodenal descending portal was on the lower left side of the duodenal bulb.Conclusion Lake of mucin in gastric antrum while not in gastric fundus,duodenal descending portal on the lower left of the duodenal bulb are the important and stably characteristics for the diagnosis of dextrogastria by gastroscopy.Gastricfunds on the right of the cardia,the left side of the gastral cavity direction are the specific but liable to variation characteristics.
