摘 要:目的探讨^(18)F-FDG PET/CT对nm23表达的非小细胞肺癌(NSCLC)分期和不同医师靶区勾画的差异,并分析其放疗计划的影响。方法选取2016年5月至2018年5月间我院收治的213例nm23表达阳性的非小细胞肺癌患者为研究对象,根据患者图像检查方法的不同,将其分为CT组和^(18)F-FDG PET/CT组。CT组106例,单独采用CT检查,^(18)F-FDG PET/CT组107例,在CT组的基础上联合^(18)F-FDG PET进行检查。并根据融合图像对NSCLC进行分期,并制定放疗计划。结果与CT组相比,^(18)F-FDG PET/CT组分期改变占比40.19%,分期未改变占比59.81%(P<0.05);^(18)F-FDG PET/CT组靶区勾画大体肿瘤体积明显较CT组更小,靶区体积减少比例明显高于CT组(P<0.05);^(18)F-FDG PET/CT勾画靶区后制定的放疗计划对减少周围正常组织的受量及损伤要显著低于CT图像(P<0.05)。结论^(18)F-FDG PET/CT在nm23表达阳性的NSCLC放疗中较CT应用价值更高,有助于临床指导NSCLC的诊断和分期,同时提高放疗医师勾画靶区的准确性,确保照射剂量,同时也能够更好的保护周围正常组织。从而提高放疗效果,提高患者生存质量。Objective To investigate the difference of ^18f-fdg PET/CT on the staging of nm23 expression in non-small cell lung cancer (NSCLC) and delineation of the target area by different physicians, and to analyze the influence of the radiotherapy plan.Methods 213 patients with nm23 positive expression of non-small cell lung cancer admitted to our hospital from May 2016 to May 2018 were selected as research objects.According to the different methods of image examination of patients, all patients were divided into CT group and ^18f-fdg PET/CT group.106 cases in the CT group were examined by CT alone, and 107 cases in the ^18f-fdg PET/CT group were examined by combining ^18f-fdg PET on the basis of the CT group.According to the fusion image, NSCLC was staged, and radiotherapy plan was made.Results Compared with the CT group, the staging changes in the ^18f-fdg PET/CT group accounted for 40.19%, while the staging changes in the ^18f-fdg PET/CT group accounted for 59.81%( P <0.05).The tumor volume of the ^18f-fdg PET/CT group was significantly smaller than that of the CT group, and the proportion of the tumor volume reduction was significantly higher than that of the CT group( P <0.05).The radiotherapy plan developed after the delineation of the target area by 18 f-fdg PET/CT was significantly lower than the CT image in reducing the amount and damage of surrounding normal tissues( P <0.05).Conclusion ^18f-fdg PET/CT had higher application value than CT in radiotherapy of NSCLC with positive nm23 expression, which was helpful for clinical guidance of diagnosis and staging of NSCLC, improving the accuracy of radiologists in delineating the target area, ensuring the radiation dose and better protecting the surrounding normal tissues.Thus, the effect of radiotherapy was improved and the quality of life was improved.
