不同磨牙拔除方式矫治安氏Ⅰ类错(牙合)的客观评价

摘　　要：目的:评价和比较单侧磨牙拔除、双侧磨牙拔除和4个前磨牙拔除方式矫治错(牙合)畸形的疗效。方法:纳入70例正畸矫治后并保存有治疗后模型和曲面体层片的患者,根据不同拔牙模式分为3组。结果:3组正畸后OGS指数在前磨牙拔除组(n=40)较低(P<0.05), 3组在上颌后牙排列、边缘嵴高度和覆盖中的差异有统计学意义,其中双侧磨牙拔除(n=12)和前磨牙拔除组在上颌后牙排列和覆盖中的差异具有统计学意义,单侧磨牙拔除组(n=18)和前磨牙拔除组在边缘嵴高度、覆盖和邻牙接触关系中的差异具有统计学意义。结论:3种拔牙矫治方式均能取得良好的疗效。前磨牙拔除组矫治效果优于磨牙拔除组。Objective:To quantitatively evaluate and to compare orthodontic outcomes among different molar extraction protocols for AngleⅠmalocclusion.Methods:70 cases of posttreatment records including dental casts and panoramic radiographs were divided into unilateral molar extraction,bilateral molar extraction and 4 premolar extraction groups.The treatment outcomes were compared among the 3 groups.Results:OGS indexes of premolar extraction group(n=40)were lower than those of other 2 groups(P<0.05)after treatment for maxillary posterior teeth alignment,marginal ridge height and over jet.Statistically significant differences were found in maxillary posterior teeth alignment and overjet between the bilateral molar extraction(n=12)and premolar extraction groups,and there were statistically significant differences for marginal ridge height,overjet and interproximal contact measurements between the unilateral molar extraction(n=18)and the premolar extraction groups.Conclusion:The 3 kinds of tooth extraction protocols are effective for achieving good orthodontic treatment outcomes,4 premolar extraction protocal is puperior to molar extraction.
