不同龈下菌斑清除方式对龈沟液中细胞凋亡因子、炎症因子及MMPs/TIMPs平衡的影响

摘　　要：目的:探讨手器刮治和喷砂清除龈下菌斑对龈沟液中细胞凋亡、炎症反应及MMPs/TIMPs平衡的影响。方法:将慢性牙周炎患者128例随机分为手器刮治组、喷砂组(n=64)。治疗前后用荧光定量PCR 2组患者龈沟液标本中凋亡分子表达量,用ELISA测定炎症因子、MMPs/TIMPs含量。结果:治疗前各项检测指标组间差异均无统计学意义(P>0.05)。治疗后7 d,2组Bcl-2 mRNA均升高(P<0.05),喷砂组高于手器刮治组(P<0.05)。Bax、Caspase-3 mRNA的表达量均降低(P<0.05),喷砂组低于手器刮治组(P<0.05)。龈沟液中炎症因子hs-CRP、IL-6的含量均降低(P<0.05),喷砂组低于手器刮治组(P<0.05)。IL-10的含量均升高(P<0.05),喷砂组高于手器刮治组(P<0.05)。龈沟液中MMP-2、MMP-8、MMP-13的含量均降低(P<0.05),喷砂组低于手器刮治组(P<0.05)。TIMP-1的含量均升高(P<0.05),喷砂组高于手器刮治组(P<0.05)。结论:手器刮治、喷砂刮治清除慢性牙周炎患者龈下菌斑均有效,但喷砂刮治在保护牙周组织,恢复内环境稳态方面更具优势。Objective:To study the effects of hand curettage(HC)and sand blastiting(SB)on the cell apoptosis factors,inflammatory response factors and MMPs/TIMPs balance in gingival crevicular fluid(GCF).Methods:128 cases of chronic periodontitis patients were randomly divided into 2 groups(n=64)and the subgingival plaque was removed by HC and SB respectively.The expression of apoptotic molecules in GCF was deteced by qPCR,inflammatory factors and MMPs/TIMPs factors in GCF were detected by ELISA.Results:Before treatment the expression of all the factors was not statistically different between the 2 groups(P>0.05).7 d after treatment.The expression of Bcl-2 mRNA increased in the 2 groups(P<0.05),in SB group was higher than that in HC group(P<0.05).The expression of Bax and Caspase-3 mRNA decreased(P<0.05),in SB group was lower than those in HC group(P<0.05).The content of hs-CRP and IL-6 decreased(P<0.05),in SB group was lower than that in HC group(P<0.05).IL-10 increased(P<0.05),in SB group was higher than that in HC group(P<0.05).MMP-2,MMP-8 and MMP-13 decreased(P<0.05),in SB group were lower than those in HC group.TIMP-1 increased(P<0.05),in SB group was higher than that in HC group(P<0.05).Conclusion:Hand curettage and sand blasting can remove subgingival plaque.Sandblasting can more effectively protect periodontal tissue and restore homeostasis.
