二代测序法分析左、右半结肠癌分子特征

摘　　要：目的探讨结直肠癌基因突变特征,分析左、右半结肠癌分子特征差异。方法回顾性分析2017年1月至2018年6月郑州大学第一附属医院行二代测序检测的结直肠癌患者168例的临床资料及肿瘤组织的二代测序结果,采用χ~2检验比较KRAS、TP53、APC、SMAD4、PIK3CA、FBXW7在左、右半结肠癌患者中的突变率。结果右半结肠癌患者的基因突变位点多于左半结肠癌患者,PIK3CA、FBXW7在右半结肠癌患者的突变率高于左半(均P<0.05),KRAS、TP53、APC、SMAD4在左半结肠癌患者的突变率高于右半(均P<0.05)。结论右半结肠癌患者的基因突变较多,PIK3CA、FBXW7、KRAS、TP53、APC、SMAD4在左、右半结肠癌的基因突变率存在差异。Objective To investigate the characteristics of gene mutations in colorectal cancer,and analyze the differences in molecular characteristics between the left and right colon cancers.Methods The clinical data and genetic test results of 168 patients with colorectal cancer with NGS in the First Affiliated Hospital of Zhengzhou University from January 2017 to June 2018 were retrospectively analyzed,and the differences in mutation rates of KRAS,TP53,APC,SMAD4,PIK3CA,and FBXW7 in left and right colon cancer were compared byχ2 test.Results The right colorectal cancer had more genetic mutations than the left,the mutation rates of PIK3CA and FBXW7 in patients with right colon cancer were significantly higher than those in the left(all P<0.05),while KRAS,TP53,APC and SMAD4 mutation in left colorectal cancer much higher than the right(all P<0.05).Conclusion The right colorectal cancer have more genetic mutations than the left and there are differences in PIK3CA、FBXW7、KRAS、TP53、APC and SMAD4 mutations in different primary sites of tumors.
