不同方式球囊扩张治疗环咽肌失弛缓所致吞咽障碍的疗效研究

摘　　要：目的研究不同方式球囊扩张治疗环咽肌失弛缓引起吞咽障碍的临床疗效。方法入选此次研究80例吞咽困难患者均为我院2017年7月～2018年4月收治,随机分为Ⅰ组(14号导尿管)、Ⅱ组(14号改良双硅胶导管)及Ⅲ组(22号导尿管),对其治疗效果进行分析。结果 (1)Ⅰ组、Ⅲ组患者治疗后洼田吞咽能力、口腔及咽部评分均优于Ⅱ组患者,差异有统计学意义(P <0.05);Ⅰ组、Ⅲ组患者治疗后洼田吞咽能力、口腔及咽部评分,差异无统计学意义(P> 0.05)。(2)Ⅰ组、Ⅲ组患者球囊注水后球囊周长、球囊直径与囊内压力均高于Ⅱ组患者,差异有统计学意义(P <0.05);Ⅰ组与Ⅲ组患者球囊注水后球囊周长、球囊直径与囊内压力,差异无统计学意义(P> 0.05)。结论球囊扩张治疗环咽肌失弛缓所致吞咽障碍的疗效较为理想,可根据患者的实际情况选取合适的治疗方式。Objective To study the clinical effect of different ways of balloon dilatation in the treatment of dysphagia caused by cricopharyngeal achalasia. Methods 80 patients with dysphagia in the Second People’s Hospital of Foshan from July 2017 to April 2018 were selected. They were randomly divided into Group Ⅰ(with No.14 urethral catheter),Group Ⅱ(with No.14 modified double silicone catheter) and Group Ⅲ(with NO.22 urethral catheter).The therapeutic effect was analyzed. Results(1)The Watian swallowing ability, oral and pharyngeal scores of Group Ⅰand Group Ⅲ were better than those of Group Ⅱ after treatment, with significant difference(P < 0.05);there was no significant difference in Watian swallowing ability, oral and pharyngeal scores between Group Ⅰ and Group Ⅲ after treatment(P > 0.05).(1)The perimeter, diameter and pressure of balloon with water injection in Group Ⅰ and GroupⅢ were higher than those in Group Ⅱ(P < 0.05). There was no significant difference in perimeter, diameter and pressure between Group Ⅰ and Group Ⅲ(P > 0.05). Conclusion The therapeutic effect of balloon dilatation in the treatment of dysphagia caused by cricopharyngeal achalasia is ideal,and the appropriate treatment method can be selected according to actual situation of patients.
