摘　　要：目的评价事件相关电位P300、TCD检测对酒精所致精神障碍患者认知功能。方法方便选取该院在2016年7月—2017年7月收治的130例确诊为由酒精所致的精神障碍患者进行研究,P300检测研究组130例酒精所致精神障碍患者,100例正常对照组,对比分析两组之间的P300结果。采用TCD检测研究组130例酒精所致精神障碍患者,100例正常对照组。分析颅内各血流参数变化,P300潜伏期、波幅作对比分析。结果 P300检测酒精所致精神障碍患者的P300潜伏期比对照组明显延长,波幅异常(t=15.258,P<0.05),TCD检测研究组总异常率78.46%,符合血管狭窄、流速加快102例,流速正常28例。对照组总异常率13.00%,两组异常率对比差异有统计学意义(χ~2=7.581,P<0.05)。TCD血流速度与P300潜伏期具有相关性。结论经该文研究发现,P300、TCD联合检测酒精所致精神障碍的早期发生是评判的良好指标,为酒精所致精神障碍的早期发生与发展提供了可靠依据确诊率和诊断率较高,有助于后期临床治疗进而提高患者生活质量。TCD联合P300诊断方法在酒精所致精神障碍患者的临床诊断上具有推广和应用价值。Objective To evaluate the cognitive function of patients with mental disorders caused by alcohol by evaluating event-related potentials P300 and TCD. Methods A total of 130 patients with mental disorders diagnosed by alcohol were convenient enrolled in our hospital from July 2016 to July 2017. P300 test group 130 patients with alcohol-induced mental disorders and 100 normal in the control group. The P300 results between the two groups were compared and analyzed. TCD was used to detect 130 patients with alcohol-induced mental disorders and 100 normal controls. The changes of blood flow parameters in the brain were analyzed, and the latency and amplitude of P300 were compared for analyzed. Results The P300 latency of P300 patients with alcohol-induced mental disorders was significantly longer than that of the control group,with abnormal amplitude(t=15.258, P<0.05). The total abnormal rate of TCD detection group was 78.46%, which was consistent with vascular stenosis and velocity increase in 102 cases. 28 cases were normal. The total abnormal rate of the control group was 13.00%, and there was a significant difference between the two groups(χ^2=7.581, P<0.05). The TCD blood flow velocity is correlated with the P300 latency. Conclusion The results of this study found that the early detection of P300 and TCD combined with alcohol-induced mental disorders is a good indicator of judging, providing a reliable basis for the early occurrence and development of alcohol-induced mental disorders. The definite diagnosis rate and diagnostic rate are high, which helps later clinical treatment, the quality of life of patients is improved. TCD combined with P300 diagnostic method has a promotion and application value in the clinical diagnosis of alcohol-induced mental disorders.
