乳酸脱氢酶、腺苷脱氨酶、癌胚抗原、总蛋白、C反应蛋白对鉴别结核性胸腹水与恶性胸腹水的临床意义分析

摘　　要：目的探讨乳酸脱氢酶、腺苷脱氨酶、癌胚抗原、总蛋白、C反应蛋白对鉴别结核性胸腹水与恶性胸腹水的临床意义。方法选取50例结核性胸腹水患者作为结核组,选取50例恶性胸腹水患者作为恶性组。比较两组患者的乳酸脱氢酶、腺苷脱氨酶、癌胚抗原、总蛋白、C反应蛋白水平。结果结核组患者腺苷脱氨酶(56.66±30.22)U/L、总蛋白(49.09±30.99)g/L、C反应蛋白(33.33±25.55)mg/L高于恶性组的(16.66±10.22)U/L、(34.09±11.29)g/L、(19.23±7.95)mg/L,乳酸脱氢酶(422.22±256.66)U/L、癌胚抗原(1.44±0.67)μg/L低于恶性组的(562.20±256.26)U/L、(35.44±11.27)μg/L,差异均具有统计学意义(P<0.05)。结论乳酸脱氢酶、腺苷脱氨酶、癌胚抗原、总蛋白、C反应蛋白可以对结核性胸腹水与恶性胸腹水进行鉴别,不仅可以诊断疾病还可以判断预后。Objective To discuss the clinical significance of lactate dehydrogenase, adenosine deaminase, carcinoembryonic antigen, total protein and C-reactive protein in differentiating tuberculous pleural and ascites from malignant pleural and ascites. Methods There were 50 patients with tuberculous pleural and ascites as tuberculous group, and 50 patients with malignant pleural and ascites as malignant group. The lactate dehydrogenase, adenosine deaminase, carcinoembryonic antigen, total protein and C-reactive protein was compared in two groups. Results Tuberculous group had higher adenosine deaminase as(56.66±30.22) U/L, total protein as(49.09±30.99) g/L and C-reactive protein as(33.33±25.55) mg/L than(16.66±10.22) U/L,(34.09±11.29) g/L and(19.23±7.95) mg/L in malignant group, and lower lactate dehydrogenase as(422.22±256.66) U/L and carcinoembryonic antigen as(1.44±0.67)μg/L than(562.20±256.26) U/L and(35.44±11.27)μg/L in malignant group. Their difference was statistically significant(P<0.05). Conclusion Lactate dehydrogenase, adenosine deaminase, carcinoembryonic antigen, total protein and C-reactive protein can differentiate tuberculous pleural and ascites from malignant pleural and ascites, and they can not only diagnose diseases but also judge prognosis.
