电针联合逍遥散对围绝经期轻中度抑郁症患者疗效及血清ACTH、CORT的影响

摘　　要：为了观察电针联合逍遥散对围绝经期轻中度抑郁症患者的临床疗效及血清促肾上腺皮质激素(ACTH)、肾上腺皮质酮(CORT)水平的影响,将60例围绝经期轻中度抑郁症患者随机分为电针组、逍遥散组、电针联合逍遥散组,电针组选取"百会"、"神门"等穴给予电针治疗,逍遥散组给予逍遥散口服治疗,电针联合逍遥散组在逍遥散组基础上加用电针治疗,三组均持续治疗1个月,比较三组患者临床疗效,检测围绝经期轻中度抑郁症患者血清ACTH、CORT水平及不良反应的情况。结果表明,电针联合逍遥散组总有效率为95. 00%,高于电针组65. 00%与逍遥散组70. 00%(P<0. 05);治疗后三组患者HAMD评分显著低于治疗前(P<0. 05),且电针联合逍遥散组HAMD评分显著低于电针组与逍遥散组(P<0. 05);治疗后血清ACTH、CORT水平均显著低于治疗前(P<0. 05),且电针联合逍遥散组患者血清ACTH、CORT水平均显著低于电针组与逍遥散组(P<0. 05);电针组、逍遥散组、电针联合逍遥散组治疗后不良反应发生率相比,差异无统计学差异(P>0. 05)。电针联合逍遥散治疗可显著改善围绝经期轻中度抑郁症患者抑郁程度,下调ACTH、CORT表达水平,值得进一步推广使用。In order to observe the clinical effect of electroacupuncture combined with Xiaoyaosan on perimenopausal depression patients and the influences on serum adrenocorticotropic hormone(ACTH) and corticosterone(CORT),60 patients with perimenopausal depression were randomly divided into electroacupuncture group, Xiaoyaosan group, and electroacupuncture combined Xiaoyaosan group, electroacupuncture group was treated with electroacupuncture on Baihui,Sishencong,and other point,Xiaoyaosan group was given Xiaoyaosan orally,electroacupuncture combined with Xiaoyaosan group was treated with electroacupuncture combined Xiaoyaosan group,the three groups were treated for 1 month,the clinical efficacy of the three groups was compared,the serum ACTH,CORT levels and adverse reactions in patients with perimenopausal depression were detected. The results show that the total effective rate of electroacupuncture combined with Xiaoyaosan group was 95. 00 %,which was higher than 65. 00 % of electroacupuncture group and70. 00 % of Xiaoyaosan group 70. 00 %(P<0. 05);after treatment,the HAMD score of the three groups was significantly lower than that before treatment(P <0. 05),and the HAMD score of the electroacupuncture combined with Xiaoyaosan group was significantly lower than that of the electroacupuncture group and Xiaoyaosan group(P < 0. 05);after treatment,the levels of serum ACTH and CORT were significantly lower than those before treatment(P<0. 05),and the levels of ACTH and CORT in electroacupuncture combined with Xiaoyaosan group were significantly lower than those in electroacupuncture group and Xiaoyaosan group(P <0. 05);there was no significant difference in the incidence of adverse reactions between electroacupuncture group,Xiaoyaosan group, and electroacupuncture combined Xiaoyaosan group(P > 0. 05). Electroacupuncture combined with Xiaoyaosan can significantly improve the depression degree of perimenopausal depression patients,down-regulate the expressions of serum ACTH and CORT,which is worthy of further promotion and use
