摘　　要：目的:探讨综合干预治疗对社区阿尔兹海默病(AD)患者疗效的影响。方法:采用随机对照方法,将100例患者随机分为两组,每组各50例,均依照病情使用常规药物治疗;同时给予干预组综合干预治疗1年,包括心理社会干预和生活方式干预。于干预前、干预6个月及1年时采用临床痴呆评定量表(CDR)、日常生活能力量表(ADL)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)对入组者进行评估和比较。结果:经过综合干预1年后,干预组CDR评分(1.78±0.89)低于对照组(2.16±0.65),差异有统计学意义(t=3.96,P<0.05);ADL评分(28.13±7.71)低于对照组(32.91±6.58),差异有统计学意义(t=7.59,P<0.01);HAMA评分(9.88±6.00)低于对照组(13.87±6.11),差异有统计学意义(t=5.22,P<0.05);HAMD量表评分(8.23±4.46)低于对照组(11.63±5.24),差异有统计学意义(t=4.76,P<0.05)。结论:综合干预治疗能够延缓社区AD患者认知功能减退的进程,改善患者的日常生活能力,减轻患者的焦虑和抑郁情绪。Objective: To explore the effect of comprehensive intervention therapy in Alzheimer's disease outpatients. Method: One hundred outpatients with Alzheimer's disease were randomly divided into the research group and control group,50 cases in both group.Both groups were given conventional treatment,and the research group was also treated with comprehensive intervention for one year.Efficacy was assessed by CDR,ADL,HAMA and HAMD before and after intervention in both groups. Results: After the comprehensive intervention,the average total score of the CDR in research group(1.78±0.89) was lower than that of control group(2.16±0.65),showing statistically group difference( P <0.05).The average total score of the ADL was statistically lower in research group(28.13±7.71) compared with control group(32.91±6.58)( P <0.01).The average total score of the HAMA in research group(9.88±6.00)was lower than control group(13.87±6.11).there was statistically difference on the HAMA( P <0.05).The average total score of the HAMD in research group(8.23±4.46)was lower than control group(11.63±5.24).there was statistically difference on the HAMD( P <0.05). Conclusion: Comprehensive intervention therapy can delay the progress of cognitive decline,improve daily living ability,and also alleviate anxiety and depression in outpatients with Alzheimer's disease.
