高频重复经颅磁刺激联合度洛西汀在重度抑郁发作急性期的疗效

摘　　要：目的:探讨高频重复经颅磁刺激(HF-rTMS)联合度洛西汀在重度抑郁发作急性期的疗效与安全性。方法:64例重度抑郁发作急性期患者随机分为研究组和对照组;两组在服用度洛西汀肠溶胶囊(60 mg/d)基础上分别接受真或伪HT-rTMS治疗(频率10 Hz,每次刺激3 000个脉冲,运动阈值100%),每周5次共3周。两组分别在入组时、治疗第1、2、3、8周末接受汉密尔顿抑郁量表(HAMD-17)、临床大体印象量表(CGI)和治疗过程中出现的症状量表(TESS)评估。结果:治疗第1周末研究组HAMD-17得分显著低于基线(t=4.132,P<0.05)及对照组(t=2.945,P<0.05);第3周末两组间比较差异有统计学意义(t=2.439,P<0.05);第3周末有效率显著优于对照组(46.7%vs 19.4%,χ~2=5.161,P<0.05);临床痊愈率两组差异无统计学意义(20%vs 6.5%,χ~2=1.411)。结论:HF-rTMS联合度洛西汀在重度抑郁发作急性期能够加速并增加抗抑郁治疗效果。Objective: To investigate the efficacy and safety of high frequency repeated transcranial magnetic stimulation(HF-rTMS) combined with luoxiting in acute stage of severe depressive episode. Method: 64 acute stage patients with severe depressive episode were randomly divided into study group and control group.On the basis treatment of duroxetine(60 mg/d),the two groups received true or false respectively HT-rTMS(frequency 10 Hz,3,000 pulses per stimulus,movement threshold 100%,5 times per week for 3 weeks).The patients were evaluated by the Hamilton depression scale(HAMD-17),clinical global impressions(CGI),treatment emergent symptom scale(TESS) on the baseline,and the end of the 1st,2ed,3rd,8th week after treatment. Results: The HAMD-17 score of the the study group at the 1st and 3rd weekend after treatment were significantly lower than the baseline and the control group(all <0.05).The efficiency at the 3rd weekend after treatment was significantly better than the control group(46.7% vs 19.4%,χ^2=5.161,P <0.05).There was no significant difference of remission rate between two groups. Conclusion: HF-rTMS combined with luoxiting can accelerate and increase the effect of antidepressant therapy in the acute stage of severe depressive episode.
