摘　　要：目的探讨Barrett食管24 h pH测定及内镜下氩气刀治疗对食管功能的影响。方法选择医院收治的胃食管反流病患者171例,根据是否存在Barrett食管分为Barrett食管组(56例)和无Barrett食管组(药物组,115例)。Barrett食管组行内镜下氩气刀治疗,并口服质子泵抑制剂(PPI),药物组口服PPI。治疗前后均行食管pH监测及食管测压,以评价食管功能。结果治疗后,barrett食管组和药物组症状评分、食道p H监测、反流时间持续≥5 min次数、Demeester积分均较本组治疗前有明显改善(P <0. 05),但上述指标和食管测压组间均无明显差异(P> 0. 05)。结论与无Barrett食管胃食管反流病患者相比,Barrett食管患者并无特征性食管功能改变,其作用机制仍需进一步研究。内镜下氩气刀治疗对食管动力及酸反流无明显影响。Objective To investigate the effect of 24 h pH monitoring and endoscopic argon beam coagulator therapy for Barrett′s esophagus on esophageal function.Methods Totally 171 patients with gastroesophageal reflux disease(GERD)admitted to our hospital were selected and divided into the Barrett′s esophagus group(56 cases)and non-Barrett′s esophagus group(drug group,115 cases)according to the presence of Barrett′s esophagus or not.The patients in the Barrett′s esophagus group underwent argon beam coagulator therapy and took proton pump inhibitor(PPI)orally.The patients in the drug group took PPI orally.Esophageal pH monitoring and esophageal manometry were performed before and after treatment to evaluate esophageal function.Results After treatment,the symptoms score,esophageal pH monitoring,times of reflux duration≥5 min and Demeester score of Barrett′s esophagus group and drug group were significantly improved compared with those before treatment(P<0.05),but there was no significant difference in the above indexes and esophageal manometry between the two groups(P>0.05).Conclusion Compared with patients without Barrett′s GERD,the patients with Barrett′s esophagus have no characteristic changes in esophageal function,and its mechanism needs further study.Endoscopic argon plasma coagulation therapy had no significant effect on esophageal motility and acid reflux.
