血清miR-574-5p和miR-10b在乳腺癌中的表达及临床意义

摘　　要：目的观察血清miR-574-5p和miR-10b在乳腺癌中的表达及其临床意义。方法乳腺癌患者75例为乳腺癌组。选择同期在我院行乳腺纤维腺瘤治疗的患者45例为乳腺纤维腺瘤组,健康体检者20例为对照组。3组血清采用实时荧光RT-PCR的方法检测miR-574-5p和miR-10b。观察乳腺癌患者血清miR-574-5p和miR-10b表达与治疗后,临床指标和相互之间的相关性及诊断乳腺癌的诊断价值。结果乳腺癌组的血清miR-574-5p和miR-10b水平明显高于乳腺纤维腺瘤组和对照组(P<0.01),乳腺纤维腺瘤组明显高于对照组(P<0.01),乳腺癌根治术后血清水平较术前明显降低(P<0.01)。乳腺癌患者血清miR-574-5p和miR-10b表达水平与绝经年龄和肿瘤大小无明显相关性(P>0.05),而与淋巴结转移,ER,PR,HER-2,病理分级,病理分期和脉管癌栓是否具有明显相关性(P<0.01)。乳腺癌患者血清miR-574-5p水平与miR-10b表达水平呈正相关(r=0.762,P<0.01)。联合检测血清miR-574-5p和miR-10b表达在诊断乳腺癌的灵敏度和特异性明显高于单独检测miR-574-5p(Z=2.164,P=0.030)和miR-10b(Z=2.874,P=0.004),而单独检测miR-574-5p与miR-10b表达水平比较,差异无统计学意义(Z=0.328,P=0.978)。结论 miR-574-5p和miR-10b参与了乳腺癌发生发展,对乳腺癌的诊断和预后具有重要的临床价值。Objective To investigate the expression and clinical significance of serum miR-574-5p and miR-10b in patients with breast cancer. Methods A total of 75 patients with breast cancer who were treated in our hospital from December 2015 to June 2018 were enrolled as breast cancer group,at the same time, the other 45 patients with mammary fibroadenoma who treated in our hospital were enrolled as breast fibroadenomas group,moreover, 20 healthy sunjects were used as control group.The expression levels of miR-574-5p and miR-10b were detected by real-time fluorescent RT-PCR.The expression levels of serum miR-574-5p and miR-10b were observed in the three groups, and the correlation between the expression levels and clinical indicators,as well as its diagnostic value for breast cancer were analyzed. Results The serum levels of miR-574-5p and miR-10b in breast cancer group were significantly higher than those in breast fibroadenoma group and control group ( P <0.01),which in breast fibroadenoma group were significantly higher than those in control group ( P < 0.01 ).Moreover the serum levels of miR-574-5p and miR-10b after surgery were significantly lower than those before surgery ( P <0.01).The expression levels of serum miR-574-5p and miR-10b in patients with breast cancer were not significantly correlated with menopausal age and tumor size ( P >0.05),however,which were significantly correlated with lymph node metastasis,ER,PR,HER-2,pathological grade,pathological stage and vascular tumor thrombus ( P <0.01).The serum levels of miR-574-5p were positively correlated with those of miR-10b in patients with breast cancer ( r =0.762, P <0.01).The sensitivity and specificity of combination detection of miR-574-5p and miR-10b in diagnosis of breast cancer were significantly higher than those bu simple detection of miR-574-5p ( Z =2.164, P <0.05) and miR-10b ( Z =2.874, P <0.01),however there were no significant differences in the sensitivity and specificity of simple detection between miR-574-5p and miR-10b ( Z =0.328, P >0.05).
