NPM1基因突变阳性急性髓系白血病的免疫表型和临床特征分析

摘　　要：目的:探讨NPM1基因突变阳性的急性髓性白血病的临床特点及免疫表型特征。方法:利用流式细胞术进行免疫分型检测,采用实时定量荧光PCR法检测NPM1基因、WT1基因、白血病融合基因,利用G显带技术检测患者的染色体核型。NPM1基因突变阳性染色体正常AML患者28例,基因及染色体均正常的AML患者41例。结果:NPM1+的急性髓细胞白血病与NPM1-的急性髓细胞白血病的发病在性别分布上有统计学差异(P <0. 05)。发病时的白细胞数、血红蛋白量及血小板数差别无统计学意义(P> 0. 05)。NPM1+的急性髓细胞白血病的CD34、CD117、HLA-DR表达率低于NPM1-的急性髓细胞白血病(P <0. 05)。NPM1基因突变阳性的AML患者WT1的表达水平高于NPM1-的急性髓细胞白血病(P <0. 05)。NPM1基因突变阳性的AML患者第1程化疗后完全缓解率与NPM1-的急性髓细胞白血病无差别(P> 0. 05)。结论:NPM1+的急性髓细胞白血病具有特定的免疫表型特征及临床特点。Objective:To explore the immunophenotypic and clinical characteristics of NPM1 mutated acute myeloid leukemia.Methods:Immunophenotyping and NPM1 gene,WT1 gene,leukemia fusion gene and chromosome karyotype were detected by flow cytometry and real time RT-PCR and G-banding technology,respectively.Acute myeloid leukemia patients with positive NPM1 mutation was 28.Gene mutation negtive and normal chromosome karyotype ones was 41.Results:There were significant difference between acute myeloid leukemia patients with positive NPM1 mutation and NPM1 mutation negtive ones at presentation in terms of sex(P<0.05).There were no differences at presentation in terms of white blood count(WBC),hemoglobin,platelet counts(PLT)(P>0.05).The expression of CD34、CD117、HLA-DR in acute myeloid leukemia patients with positive NPM1 mutation lower than that of NPM1 mutation negtive ones(P<0.05).The expression of WT1 in acute myeloid leukemia patients with positive NPM1 mutation higher than that of NPM1 mutation negtive ones(P<0.05).There were no difference at complete remission rate after first chemotherapy between acute myeloid leukemia patients with positive NPM1 mutation and NPM1 mutation negtive ones(P>0.05).Conclusions:There are specific feature in terms of immunophenotypic and clinical characteristics of NPM1 mutated acute myeloid leukemia.
