摘　　要：目的:探讨逆向调强放疗(IMRT)联合同步化疗对老年非小细胞肺癌(NSCLC)患者T淋巴细胞亚群及循环肿瘤细胞(CTC)的影响。方法:将2016年10月至2017年10月江苏省启东市人民医院收治的老年NSCLC患者90例随机分为观察组和对照组,每组45例。对照组仅采用化疗,观察组采用IMRT联合同步化疗。对比两组治疗效果、生活质量、血清肿瘤标志物指标(CA125、CA153、CA199、CEA)、T淋巴细胞亚群(CD_4^+、CD_3^+、CD_8^+、CD_(29)^+)及CTC水平差异。结果:观察组治疗有效率、生活质量改善率均明显高于对照组(均P<0.05)。观察组患者治疗后CA125、CA153、CA199、CEA、CD_8^+、CTC阳性率明显低于对照组(均P<0.05),CD_3^+、CD_4^+、CD_(29)^+水平明显高于对照组(均P<0.05)。结论:IMRT联合同步化疗治疗老年NSCLC患者,通过改善T淋巴细胞亚群水平,降低CTC含量,起到提高患者免疫,抑制肿瘤生长,提高临床疗效的作用。Objective: To investigate the effect of intensity-modulated radiation therapy (IMRT) combined with synchronous chemotherapy on T lymphocyte subsets and circulating tumor cells (CTC) in aged patients with non-small cell lung cancer (NSCLC). Methods: A total of 90 aged patients with NSCLC admitted to our hospital from October 2016 to October 2017 were selected,and randomized to receive either synchronous chemotherapy (n=45,control group) or IMRT combined with synchronous chemotherapy (n=45,observation group).The clinical effect,life quality,serum tumor markers (CA125,CA153,CA199,CEA),T lymphocyte subsets (CD4+,CD 3^+,CD 8^+,CD 29^+),and CTC levels between the two groups were compared. Results: The effective rate and life quality improvement rate were higher in the observation group than those in the control group (P<0.05).After treatment,the CA125,CA153,CA199,CEA and CD 8^+ levels as well as the CTC positive rate in the observation group were lower than those in the control group,while the levels of CD 3^+,CD 4^+ and CD 29^+ were significantly higher (P<0.05). Conclusion: IMRT combined with synchronous chemotherapy was effective in the treatment of elderly patients with NSCLC.It could improve the levels of T lymphocyte subsets,reduce the CTC content and inhibit tumor growth.
