血栓弹力图对卵巢恶性肿瘤患者凝血状态评估的价值分析

摘　　要：目的:探讨血栓弹力图评估卵巢恶性肿瘤患者凝血状态的价值。方法:选取2017年11月一2018年5月在我院住院治疗的卵巢肿瘤初诊患者151例,经确诊,恶性肿瘤117例,设为恶性肿瘤组,良性肿瘤34例,设为良性肿瘤组。所有患者均进行血栓弹力图检测。检测参数包括反应时间(R)、凝血形成时间(K)、血块形成速率(Angle角)、血块最大强度(MA)、血块纤溶指标(LY30)和凝血综合指数(CI).结果:与良性肿瘤组相比,恶性肿瘤组的R、K减少,Angle.MA和CI值增加,差异有统计学意义(P<0.05);两组对比,Ly30无统计学差异(P>0.05)。将肿瘤Ⅰ期与Ⅱ期合为组1,将肿瘤Ⅲ期与Ⅳ期合为组2进行比较分析,与组1相比,组2的Angle.MA和CI值增加,差异有统计学意义(P<0.05),R、K、Ly30无统计学差异(P>0.05).与良性肿瘤组相比,恶性肿瘤组患者的凝血功能异常率较高,差异有统计学意义(P<0.05).结论:血栓弹力图对评估卵巢恶性肿瘤患者凝血状态的应用价值较高。Objective To explore the value of thromboelastography in assessing coagulation status in patients with ovarian malignancies.Methods A total of 151 patients with newly diagnosed ovarian tumors who were hospitalized in our hospital from November 2017 to May 2018 were enrolled.Immediately after admission,thromboelastography was performed.Test parameters include reaction time (R),coagulation formation time (K),clot formation rate (Angle angle),maximum clot strength (MA),clot fibrinolytic index (IY30), and coagulation index (CI).Results Compared with the benign tumor group,the R and K of the malignant tumor group decreased,and the Angle, MA and CI values increased.The difference was statistically significant (P < 0.05;There was no significant difference between the two groups (P > 0.05).The tumors in stage I and stage II were combined into group 1, and the stage III and stage IV were combined into group 2 for comparative analysis.Compared with group 1, group 2 had increased Angle,MA and CI values,and the difference was statistically significant.Significance (P < 0.05,R,K,Ly30 were not statistically different (P > 0.05).Compared with the benign tumor group,the coagulation abnormality rate was higher in the malignant tumor group,the difference was statistically significant [P < 0.05). Conclusion The thromboelastography has a higher value in the evaluation of coagulation status in patients with ovarian malignancies.
