镰状韧带上组静脉开放的影像诊断

摘　　要：目的:总结肝镰状韧带上组静脉开放的影像表现及临床意义。方法:回顾性分析3例肝外肿瘤患者肝镰状韧带上组静脉开放的CT图像及临床资料。结果:3例镰状韧带上组静脉开放患者,1例造影剂经左锁骨下静脉-心包膈静脉-镰状韧带上组静脉引流至门静脉,动脉期肝左内叶可见片状显著强化区,内见造影剂分层。1例造影剂经左锁骨下静脉-左胸外侧静脉-左胸廓内静脉-镰状韧带上组静脉引流至门静脉,动脉晚期肝左内叶可见片状显著强化区,内见造影剂呈"血湖"状。行图像后处理三维重建,有助于直观显示异常。1例CT示扩张迂曲之门静脉左支经镰状韧带上组静脉-左侧胸廓内静脉引流至上腔静脉。结论:胸腹部CT增强结合三维重建,能更好地显示镰状韧带上组静脉开放,正确认识这些异常有一定的临床意义。Objective: To summarize the imaging manifestations and clinical significance of the superior group of veins in the falciform ligament . Methods: CT images and clinical data of 3 patients with extrahepatic tumor with the superior group of veins in the falciform ligament were analyzed retrospectively. Results: Case 1 :Contrast agents were from the left subclavian vein - pericardium phrenic vein - the superior group of veins in thefalciform ligament to the portal vein.Flake reinforced region was found intheleft medial hepatic lobe in hepatic arterial phasesignificantly. Case2 :Contrast agents were from the left subclavian vein - left lateral thoracic vein - the left internal thoracic vein - the superior group of veins in the falciform ligament to the portal vein. Blood-lakeenhancementwas found intheleft medial hepatic lobe in hepatic arterial phasesignificantly.Case3 : CT showed that the left branch of the dilated portal vein was draining from the left thoracic vein to the inferior vena cava through the superior group of veins in the falciform ligament. Conclusion: CT enhancement combined with 3D reconstruction can better show the superior group of veins in the falciform ligament.
