摘 要:目的探讨婴幼儿支原体肺炎(mycoplasma pneumoniae pneumonia,MPP)免疫球蛋白及T淋巴细胞亚群表达水平变化情况。方法分别采集68例MPP患儿(MPP组)和同期健康体检的68例婴幼儿(对照组)静脉血3 mL,检测IgG、IgA、IgM、C_3、C_4和T淋巴细胞亚群比例。MPP患儿采集时间分别在急性期抗感染治疗前(入院后2 h内)和应用大环内酯类抗生素治疗2~4周后(临床及影像学症状均显著好转)。结果对照组和MPP组恢复期补体C_3、C_4水平均高于MPP组急性期,MPP组急性期IgA、IgM水平均显著高于MPP组恢复期,差异均有统计学意义(P<0.05)。三组血清IgG水平比较差异无统计学意义(P>0.05)。MPP组急性期CD_4^+T和CD_4^+/CD_8^+T均显著低于对照组和MPP组恢复期,MPP组急性期CD_8^+T显著高于对照组和MPP组恢复期;MPP组恢复期CD_8^+T高于对照组,但CD_4^+/CD_8^+T低于对照组;差异均有统计学意义(均P<0.05)。结论婴幼儿MPP急性期及MPP恢复期体液免疫和细胞免疫功能均有所降低。Objective To recognize the pattern of changes in the levels of expression of immunoglobulin and T lymphocyte subsets in infants with mycoplasma pneumonia (MPP). Methods Firstly, 68 infants with MPP were taken as the experimental group of study and 68 infants diagnosed by physical examination as healthy subjects were chosen as the control. Secondly, 3 mL of venous blood collected from each subject for analysis. The ratio of IgG, IgA, IgM, C 3 , C 4 and T lymphocyte subsets were examined. The times for collection of venous blood from the experimental group was set before the acute anti-infective treatment, within 2 hours after admission and after treatment with macrolide antibiotics for 2 to 4 weeks when both clinical and imaging symptoms were significantly improved. Results The levels of complement C 3 and C 4 in both groups as observed during the recovery phase were found to be higher than that of the experimental group as observed in the acute phase. The IgA and IgM levels of the experimental group in the acute phase were significantly higher than those observed in the recovery phase. The differences were statistically significant ( P <0.05). No significant difference was found between the groups in serum IgG levels ( P >0.05). The levels of CD 4 + T and CD 4 +/CD 8 + T in the experimental group as observed during the acute phase was lower than those in the control group and those in the experimental during recovery phase. The level of CD 8 + T in the experimental during acute phase was higher than that in the control and that in the experimental during recovery phase ( P <0.05). The level of CD 8 + T as observed in the experimental during recovery phase was higher than that in the control. However, the level of CD 4 +/CD 8 + T was lower than that in the control group. The differences were statistically significant ( P <0.05). Conclusions The humoral and cellular immune functions of infants in acute and convalescent stages of MPP decreased.
