盆腹部侵袭性血管黏液瘤影像表现分析及文献回顾

摘　　要：目的回顾性分析盆腹部侵袭性血管黏液瘤(AAM)的临床和影像表现.进-步提高对该病的认识。方法收集盆腹部AAM患者的临床和影像资料,并对PubMed和中国生物医学数据库进行系统检索和筛选。对本组病例和文献报道的病例进行汇总,综合分析其临床及影像学特征。结果按照纳入和排除标准.最终纳入文献23篇.共61例.加上本组1例.共62例,其中男4例,女58例,年龄20~75岁.平均(39.0±1.2)岁。发病部位:单纯盆腔25例、单纯会阴部11例、阴囊1例、盆腔及会阴部均累及25例.CT表现为囊实性密度影9例.增强CT含结节强化血管6例.漩涡征或分层征21例;MRI表现T.为稍低信号.「.为稍高信号,增强内呈分层状或漩涡状41例。结论漩涡征或分层征、增强血管样强化结节是AAM重要影像征象,有助于提高该病的术前诊断及复发病变的检出。Objective to analyze clinical and imaging manifestations of aggressive angiomyxoma (AAM) in the pelvis and perineum in order to make a further understanding of this disease.Methods The medical records of patients with AAM in the pelvis and perineum were reviewed from the electronic medical record system. PubMed and Chinese biomedical databases were also searched carefully.The clinical and imaging features of cases were analyzed together. Results According to inclusion and exclusion criteria,23 references were included. 62 cases (4 males and 58 females) of AAM in the pelvis and abdomen were included,with 1 in our hospital and 61 cases in the literature.The average age was 39.0±1.2 ( range: 20 - 75 years).The tumors located in the pelvic cavity for 25 cases, in the perineum for 11 cases, in the scrotum for 1 case,in the pelvic cavity and perineum for 25 cases.Cl' images showed tumors with cystic and solid components in 9 cases,nodular enhancement in 6 cases,and swirling strands or layering internal pattern in 21 cases;while MRI images showed a slightly hyper-intensity lesion on T[ and a slightly hyper-intensity lesion on T2. lesions showed swirling strands or layering internal pattern on MRI enhancement in 11 cases.Conclusion The swirling strands or layering internal pattern have very important imaging value in the diagnosis and the detection of recurrent lesions.
