摘 要:目的研究观察计算机辅助制造(computer aided design-computer aided manufacturing,CAD-CAM)个性化基台与成品基台在后牙区种植修复食物嵌塞的效果并对其相关性进行分析。方法取30例后牙区种植修复患者的共计50颗种植体,将患者分为实验组和对照组,各25颗。实验组给予CAD-CAM个性化基台在后牙区种植修复,对照组给予成品基台进行修复。观察并记录2组治疗3个月后复诊情况,依照牙龈乳头指数(PIS)对2组龈乳头形态的差别进行评价,并记录2组食物嵌塞的发生情况,探讨并分析龈乳头形态与食物嵌塞的相关性。结果2组患者后牙区种植修复后3个月复诊情况差异无统计学意义(P>0.05)。实验组龈乳头形态良好比例显著高于对照组,龈乳头形态不良比例显著低于对照组,差异有统计学意义(P<0.01)。实验组后牙区种植修复3个月后食物嵌塞比例显著低于对照组,差异有统计学意义(P<0.01)。后牙区种植修复3个月后龈乳头形态良好食物嵌塞比例显著低于龈乳头形态不良食物嵌塞比例,差异有统计学意义(P<0.01)。结论采用CAD-CAM个性化基台对后牙区种植修复,有利于形成更好的龈乳头形态,减少食物嵌塞,有效促进后牙区种植体周围组织的健康生长,值得临床广泛推行。Objective To investigate the effect of computer-aided design-computer-aided manufacturing(CAD-CAM)personalized abutment and finished abutment on the restoration of food impaction in posterior teeth area and to analyze the correlation between gingival papilla morphology and food impaction.Methods A total of 50 dental implants were selected from 30 patients with dental implant prosthesis.The patients were divided into an experimental group and a control group,25 implants in each group.The experimental group was implanted with CAD-CAM individualized abutment in the posterior teeth area,while the control group was implanted with finished abutment.The follow-up visits of the experimental group and the control group after 3 monthsztreatment were recorded.According to the gingival papilla index(PIS),the difference of gingival papilla morphology between the two groups was evaluated,and the occurrence of food impaction in the two groups was recorded.The correlation between gingival papilla morphology and food impaction was discussed and analyzed.Results There was no significant difference between the two groups at 3 months after implant restoration(P>0.05).The proportion of good shape of gingival papilla in the experimental group was significantly higher than that in the control group,and the proportion of bad shape of gingival papilla was significantly less than that in the control group(P<0.01).The number of food impaction in the experimental group was significantly less than that in the control group after 3 months of implant restoration in the posterior teeth area(P<0.01).The number of food impaction in patients with good shape of gingival papilla was significantly less than that in patients with bad shape of gingival papilla 3 months after implant restoration in posterior teeth area(P<0.01).Conclusions In the repair of posterior dental implant,the use of CAD-CAM customized abutment is conducive to the formation of better gingival papilla shape,reduce food impaction,and effectively promote the healthy growth of tissue
