摘　　要：目的探讨脆性组氨酸三联体(FHIT)和鼠双微体蛋白2(MDM2)蛋白在膀胱尿路上皮癌中的表达及意义。方法采用链霉菌抗生物素蛋白-过氧化物酶连结法检测FHIT和MDM2蛋白在72例膀胱尿路上皮癌和42例正常膀胱尿路上皮中的表达情况,并对两项指标的表达情况与膀胱尿路上皮癌临床病理特征的关系进行分析。结果 FHIT蛋白在膀胱尿路上皮癌中的阴性表达率为58.33%,在正常膀胱尿路上皮中为4.76%,两者比较差异有统计学意义(P<0.05)。有吸烟史的FHIT蛋白阴性表达率为86.84%,无吸烟史的为29.41%,两者比较差异有统计学意义(P<0.05)。FHIT蛋白阴性表达率与膀胱尿路上皮癌分化程度、TNM分期、淋巴结转移、肿瘤大小、吸烟、年龄均有密切关系(均P<0.05),而与性别无关(P>0.05)。MDM2蛋白在膀胱癌中阳性表达率63.89%,在正常膀胱尿路上皮中为7.69%,两者比较差异有统计学意义(P<0.05)。MDM2蛋白阳性表达与肿瘤分化程度、TNM分期、淋巴结转移、肿瘤大小、吸烟均有关(均P<0.05),与年龄、性别均无关(均P>0.05)。在膀胱尿路上皮癌中,FHIT与MDM2蛋白表达呈负相关(r=-0.816,P<0.05)。结论 FHIT阴性表达和MDM2蛋白阳性表达可能促进了膀胱癌恶性增殖及转移,联合检测能为判断膀胱癌的恶性程度及转移潜能提供参考。Objective To investigate the expression and significance of fragile histidine triad(FHIT)and murine double minute 2(MDM2)protein in bladder urothelial carcinoma.Methods FHIT and MDM2 proteins were detected by streptomyces antibiotin protein-peroxidase conjugation method in 72 samples of bladder urothelial carcinoma and 42 samples of normal bladder mucosa.The relationship between expression levels and the clinicopathological features of bladder urothelial carcinoma were analyzed.Results The negative expression rates of FHIT protein in bladder urothelial carcinoma and normal urothelial samples were 58.33%and 4.76%,respectively(P<0.05).The negative expression rate of FHIT protein was 86.84%for those with smoking history,and 29.41%for those without smoking history(P<0.05).The expression rate of FHIT protein was closely related to the pathological type,degree of cell differentiation,TNMstage and lymph node metastasis of bladder urothelial carcinoma(P<0.05),but not related to the age and gender of patients(P>0.05).The positive expression rate of MDM2 protein was 63.89%and 7.69%expressed in urothelial carcinoma and normal urothelial epithelium(P<0.05).The overexpression of MDM2 protein was significantly correlated with tumor differentiation,TNM stage,smoking history and lymph node metastasis(P<0.05),but not with age,gender(P>0.05).FHIT was negatively correlated with MDM2 protein in bladder urothelial carcinoma(r=-0.816,P<0.05).Conclusion FHIT protein loss and MDM2 protein overexpression may promote malignant proliferation and metastasis of bladder urothelial carcinoma.Combined detection of the two indicators may be used for the determination of malignant degree,metastasis potential and prognosis of bladder urothelial carcinoma.
