摘 要:目的探讨PET/CT相关摄取指标与复发性卵巢癌患者二次减瘤效果及无进展生存期(PFS)之间的关系。方法本研究纳入了2013年1月至2015年12月进行二次减瘤术的复发卵巢癌患者62例,均在术前90d内行PET/CT检查,记录PET/CT摄取指标包括最大标准摄取值(SUVmax)、代谢活跃的肿瘤体积(MTV)和病灶总糖酵解值(TLG),分析其与复发卵巢癌患者二次减瘤效果及预后的相关性。结果研究结果显示PET/CT摄取指标中MTV与TLG(P=0.012,P=0.003)与二次减瘤效果有相关性(MTV≤9.87ml及TLG≤40.83g的患者二次减瘤达到理想效果的比率高于MTV>9.87ml和TLG>40.83g的患者),但是SUVmax与肿瘤细胞减灭效果之间没有明显的相关性(P=0.267);MTV>9.87ml及TLG>40.83g的患者的PFS明显缩短(P=0.048,P=0.024),而SUVmax与患者预后之间无显著相关性(P=0.060)。结论反映肿瘤代谢负荷的PET/CT摄取指标与复发卵巢癌患者的二次减瘤效果及预后有相关性。Objective To investigate the relationship of PET/CT metrics with the effect of optimal debulking and prognosis in patients with recurrent ovarian cancer undergoing secondary cytoreductive surgery.Methods Clinical data of 62 patients with recurrent ovarian cancer undergoing secondary cytoreductive surgery in zhejiang cancer hospital between January 2013 to December 2015 were retrospectively reviewed.All patients underwent PET/CT examination within 90 days before surgery,the uptake metrics included maximum standardized uptake values(SUVmax),metabolically active tumor volumes(MTV)and total lesion glycolysis(TLG).The correlation of different PET/CT uptake metrics with the effect of secondary debulking and progression-free survival(PFS)in patients with recurrent ovarian cancer was analyzed.Results Among the uptake metrics,MTV and TLG were correlated with the effect of secondary debulking,patients with MTV≤9.87ml and TLG≤40.83g had higher rates of ideal secondary debulking than those with MTV>9.87ml and TLG>40.83g(P=0.012,P=0.003);but there was no significant correlation between SUVmax and the effect of debulking(P=0.267).Patients with a TLG≥40.83g and MTV>9.87ml had significantly shorter PFS(P=0.048,P=0.024).SUVmax was not significantly related to PFS(P=0.060).Conclusion PET/CT uptake metrics MTV and TLG that reflect metabolic tumor burden are associated with the effect of secondary debulking and prognosis in patients with recurrent ovarian cancer.
