摘 要:目的探讨乳腺癌易感基因1(BRCA1)和血管内皮生长因子(VEGF)在食管鳞癌患者中的表达情况及与预后的关系。方法选取110例行食管癌切除术的患者,收集食管鳞癌组织及癌旁正常黏膜组织,另收集58例食管上皮内瘤变患者的组织,采用免疫组织化学法比较食管鳞癌组织(n=110)、癌旁正常黏膜组织(n=110)及食管上皮内瘤变组织(n=58)中BRCA1和VEGF的表达情况,分析BRCA1、VEGF阳性表达率与临床特征及生存的关系,并采用Cox风险模型分析影响食管鳞癌预后的独立危险因素。结果食管鳞癌、食管上皮内瘤变和癌旁正常黏膜组织中,BRCA1阳性表达率分别为78.18%、46.55%和26.36%,VEGF阳性表达率分别为82.73%、32.76%和19.09%,差异均有统计学意义(P<0.01)。BRCA1和VEGF在食管鳞癌组织中的表达呈显著正相关(r=0.341,P<0.01)。不同TNM分期、分化程度及有无淋巴结转移的食管鳞癌患者BRCA1阳性表达情况比较,差异均有统计学意义(P<0.05);不同TNM分期、分化程度、浸润深度及有无淋巴结转移、远处转移的食管鳞癌患者VEGF阳性表达情况比较,差异均有统计学意义(P<0.05)。BRCA1和VEGF阴性表达患者的生存情况均优于阳性表达患者,差异均有统计学意义(P<0.05)。分化程度、淋巴结转移及VEGF表达是影响食管鳞癌患者预后的独立危险因素(P<0.01)。结论 BRCA1和VEGF在食管鳞癌组织中均表达上调,两者可能成为食管鳞癌诊断及预后评估的有效指标,但BRCA1不能作为提示食管癌预后的独立危险因素。Objective To investigate the expression of breast cancer susceptibility gene 1 (BRCA1) and vascular endothelial growth factor (VEGF) in esophageal squamous cell carcinoma and its association with prognosis. Method The study included 110 cases who were administered with esophageal cancer surgery, during which the cancer tissues and adjacent normal mucosa were collected, besides, the samples of intraepithelial neoplasia were collected from another 58 cases, and immunohistochemical staining was applied to detect the expressions of BRCA1 and VEGF in esophageal squamous cell carcinoma group (n=110), adjacent normal tissues group (n=110) and esophageal intraepithelial neoplasia group (n=58), furthermore, the relationship between the positive expression, clinicopathological features and survival were analyzed, and the Cox regression model was used to analyze the independent risk factors for the prognosis of esophageal cancer. Result The positive rate of BRCA1 were 78.18%, 46.55% and 26.36%, and the positive rate of VEGF were 82.73%, 32.76% and 19.09% in esophageal squamous cell carcinoma group, esophageal neoplasia group and adjacent normal mucosa group, respectively, the differences were statistically significant (P<0.01). The expression of BRCA1 and VEGF in esophageal carcinoma were positively correlated with each other (r=0.341, P<0.01). BRCA1 expression was associated with TNM stage, degree of differentiation and lymph node metastasis in patients with esophageal squamous cell carcinoma (P<0.05);and the positive expression of VEGF was also correlated with TNM stage, degree of differentiation, depth of invasion and lymph node metastasis and distant metastasis (P<0.05). The survival of patients with negative BRCA1 and VEGF was significantly better than that in those with positive expressions, the difference was statistically significant (P<0.05). Differentiation degree, lymph node metastasis and expression of VEGF were independent risk factors for patients with esophageal cancer. Conclusion The expression of BRCA1 and
