摘 要：目的:探讨正畸牵引配合烤瓷美容修复治疗前牙龈下冠折的临床应用效果。方法:选择笔者医院2015年5月-2016年9月收治的188例前牙龈下冠折患者为研究对象,按照患者住院ID编号尾数随机分为观察组(97例)和对照组(91例)。对照组患者切除牙龈后进行烤瓷修复,观察组患者则采用正畸牵引联合烤瓷修复治疗。对比两组患者的临床疗效、咬合功能、美观程度以及不良反应发生情况。结果:观察组总有效率为97.94%,显著高于对照组的90.11%,两组比较有统计学意义(P<0.05)。观察组治疗后的LCP-HB、LCP-MC肌电值以及PES、WES指数水平分别为(74.05±4.94)mV、(76.24±5.07)mV、(8.28±1.13)分、(8.24±1.16)分、均显著高于对照组的(69.21±4.11)mV、(71.35±4.23)mV、(7.24±0.97)分、(7.33±0.94)分,P<0.05。观察组对美容修复的总满意度为97.94%,明显高于对照组的90.11%,差异比较有统计学意义(P<0.05)。观察组患者出现牙龈充血、牙周水肿、牙齿疼痛、骨质吸收以及牙冠松动等不良反应发生率分别为5.15%、4.12%、5.15%、2.06%、1.03%,均显著低于对照组的14.29%、15.38%、17.58%、10.99%、6.59%,差异有统计学意义(P<0.05)。结论:修复治疗前牙龈下冠折,正畸牵引联合烤瓷美容修复有助于前牙功能和面部美观的恢复,同时也能较好地保存残留的牙根及牙周组织,达到了美观、牢固、舒适的要求,其修复效果优于单纯烤瓷修复,值得临床上推广应用。Objective To research the clinical application of orthodontic traction combined with porcelain cosmetic restoration in treatment of anterior subgingival crown fold. Methods From May 2015 to September 2016, 188 patients with anterior subgingival crown fracture who treated in our hospital were selected as the research objects. According to the hospitalized patients with ID number mantissa, they were randomly divided into the observation group (97 cases) and the control group (91 cases). The control group was treated with PFM after gingiva excision. The patients in the observation group were treated with orthodontic traction combined with porcelain repair. The clinical efficacy, occlusal function, aesthetic degree and adverse reaction of the two groups were compared. Results The total effective rate of the observation group was 97.94%, which was significantly higher than that of the control group (90.11%, P <0.05). The electromyographic values of LCP-HB and LCP-MC, the PES and WES index in the observation group were (74.05±4.94) mV,(76.24±5.07) mV,(8.28±1.13) score,(8.24±1.16) score, were significantly higher than those in the control group ((69.21±4.11) mV,(71.35±4.23) mV,(7.24±0.97) score,(7.33±0.94) score,P <0.05). The total satisfaction of the observation group on the cosmetic repair was 97.94%, which was significantly higher than that of the control group (90.11%), and the difference was statistically significant (P <0.05). The incidence of adverse reactions such as gingival congestion, periodontal edema, tooth pain, bone resorption and crown loosening in the observation group were 5.15%, 4.12%, 5.15%, 2.06%, 1.03%, were significantly lower than those in the control group (14.29%, 15.38%, 17.58%, 10.99%, 6.59%), and the differences were statistically significant (P <0.05). Conclusion In the treatment of anterior subgingival crown fold, orthodontic traction combined with porcelain ceramic restoration is conducive to restore the function of the anterior teeth and the beauty of the face.
