经结膜入路OF释放结合FLOO悬吊矫治下睑袋伴TTD的临床研究

摘　　要：目的回顾性分析经结膜入路眶隔脂肪(OF)释放结合眶肌筋膜韧带(FLOO)悬吊法矫治下睑袋伴泪沟畸形(TTD的临床效果。方法 60例下睑袋伴TTD患者随机分为两组,各30例。观察组行结膜入路OF释放结合FLOO悬吊法,对照组行经结膜入路OF释放术,对比两组临床治疗效果。结果术后3个月时,两组切口恢复满意度、并发症发生率比较无显著差异(P> 0.05),但观察组对下睑袋改善、下睑皱纹祛除及切口恢复的满意度评分显著高于对照组(P <0.05),Barton分级0级构成比显著高于对照组(P <0.05)。结论较单一经结膜入路OF释放矫治下睑袋伴TTD,结合FLOO悬吊法可取得更佳的手术获益,且不增加术后并发症风险。Objective To analyze the clinical effect of transconjunctival release of orbital fat(OF) combined with fascial ligament of orbit(FLOO) suspension in the treatment of lower eyelid bag with tear trough deformity(TTD). Methods A total of 60 patients with lower eyelid bags and TTD were randomly divided into two groups(n =30, respectively). The observation group was treated with transconjunctival release of OF combined with FLOO suspension, while the control group was just treated with transconjunctival release of OF. The clinical effects were compared between the two groups. Results At three months after operation,there was no significant difference in incision recovery satisfaction and complication rate between the two groups( P > 0.05), but the satisfaction score of the observation group with lower eyelid bags improvement, lower eyelid wrinkles removal and incision recovery was significantly higher than that of the control group(P < 0.05), and the composition ratio of Barton grade 0 was significantly higher than that of the control group(P < 0.05). Conclusion Compared with the treatment of lower eyelid bags with TTD only through transconjunctival approach, transconjunctival release of OF combined with FLOO suspension can achieve better surgical benefits without increasing the risk of complications.
