0.5%聚维酮碘结膜囊不同停留时间对白内障超声乳化术后早期干眼的影响

摘　　要：目的分析白内障手术前使用0.5%聚维酮碘溶液结膜囊进行冲洗消毒停留不同时间对术后早期干眼的影响。方法选取2017年1月-2018年5月在我院行双眼白内障超声乳化手术的200例患者作为研究对象,均分为2组,第一组100例患者,一眼使用0.5%聚维酮碘溶液结膜囊停留30s,另一眼使用0.8%庆大霉素稀释液冲洗结膜囊,聚维酮碘组为观察组,庆大霉素稀释液为对照组;第二组100例患者,一眼使用0.5%聚维酮碘溶液结膜囊停留60s,另一眼使用0.8%庆大霉素稀释液冲眼,聚维酮碘组为观察组,庆大霉素稀释液为对照组;分别对比聚维酮碘组与庆大霉素稀释液组术后出现干眼的情况。结果截至到术后6周,第一组,观察组和对照组干眼症的发生率对比没有统计学差异(P>0.05);第二组观察组和对照组干眼症发生率对比有统计学差异(P<0.05)。结论 0.5%聚维酮碘结膜囊停留30s不会增加白内障超声乳化术后早期干眼的发生率,而停留60s则会增加术后早期的干眼发生率。Objective To analyze the effect of using 0.5% povidone-iodine solution for rinse and disinfection of conjunctival sac with different retention time before cataract surgery on early postoperative dry eye.Methods A total of 200 patients who underwent binocular phacoemulsification in our hospital from January 2017 to May 2018 were included in the study and were divided into 2 groups. The first group of 100 patients used 0.5% povidone-iodine solution on conjunctival sac of one eye holding for 30 s, with conjunctival sac of the other eye rinsed with 0.8% gentamicin dilution. The povidone iodine group was set as observation group, and the gentamicin dilution as the control group. The second group of 100 patients used 0.5% povidone-iodine solution on conjunctival sac of one eye holding for 60 s, with conjunctival sac of the other eye rinsed with 0.8% gentamicin dilution. The povidone iodine group was set as observation group, and the gentamicin dilution as the control group. Postoperative dry eye in the povidone iodine group and the gentamicin dilution group was compared respectively. Results As of the 6th week after surgery, in the first group, there was no statistical significance concerning the difference in incidence rate of dry eye between the observation group and the control group (P>0.05);In the second group, there was statistical significance concerning the difference in incidence rate of dry eye between the observation group and the control group (P< 0.05). Conclusion Holding 0.5% povidone iodine on conjunctival sac for 30s does not increase the incidence rate of early postoperative dry eye after phacoemulsification, while holding for 60s will increase the chance of early postoperative dry eye.
