摘  要:探讨缺血性脑卒中不同时间段低剂量CT颅脑灌注参数与NIHSS评分的相关性。对2015年9月至2016年7月在四川大学华西医院神经内科住院行脑卒中CT扫描的临床及影像结果均确诊为缺血性脑卒中,并进行NIHSS评分的76例患者进行回顾性分析,按发病时间分为3组:Ⅰ组(0~3 h )25例,Ⅱ组(3~6 h) 29例及Ⅲ组(6~ 24 h ) 22例,均进行了低剂量CT颅脑灌注扫描,得到相应的CT灌注参数,包括CBF、CBV、MTT及TTP,研究每组各参数与NIHSS评分的相关性。Ⅰ组及Ⅱ组梗死病灶灌注数据CBF、CBV、TTP及MTT与NIHSS评分均无相关性( P >0.05),Ⅲ组灌注数据中CBF、CBV与NIHSS呈负相关( P <0.01)。CTP扫描中6 h内各参数与NIHSS评分无相关性,6~24 h CBF、CBV与NIHSS评分呈负相关。Objective:To investigate the correlations between low dose CT perfusion parameters and NIHSS score in different periods of ischemic stroke.Methods:From September 2015 to July 2016, 76 patients with ischemic stroke were diagnosed by clinical and imaging results. According to the time of onset, they were divided into three groups: 25 patients in group Ⅰ(0-3 h), 29 patients in group Ⅱ(3-6 h) and 22 patients in group Ⅲ(6-24 h). Low-dose CT perfusion scanning was performed to obtain the corresponding CT perfusion parameters, including CBF, CBV, MTT and TTP. The correlations between each parameters and NIHSS score were studied in the three groups.Results:CBF, CBV, TTP and MTT were not correlated with NIHSS score in groupⅠ and groupⅡ(P>0.05). CBF and CBV were negatively correlated with NIHSS score in group Ⅲ(P<0.01).Conclusion:There are no correlations between CT perfusion parameters and NIHSS score within 6 hours of ischemic stroke, but there are negative correlations between CBF, CBV and NIHSS score at 6-24 hours.
