摘 要:目的研究不同剂量自制新鲜食材匀浆膳对重症脑卒中合并2型糖尿病患者的营养支持效果。方法重症脑卒中合并2型糖尿病患者88例,随机分成4组,每组分别给予自制新鲜食材匀浆膳的剂量750 ml(约818 kcal)、1 000 ml(约1 108 kcal)、1 250 ml(约1 368 kcal)和1 500 ml(约1 875 kcal),进行为期4周的营养支持。采用自身对照方法,记录患者营养支持前后血糖和其他生化指标的改变。结果重症脑卒中合并2型糖尿病患者经过4周不同剂量自制新鲜食材匀浆膳的营养支持后,空腹血糖和餐后2 h血糖指标均有下降,差异有统计学意义(P<0.05);750 ml组的总蛋白和血红蛋白水平较营养支持前有所下降,钠离子较营养支持前有所升高,差异有统计学意义(P<0.05);1 000 ml组的钠离子较营养支持前有所升高,差异有统计学意义(P<0.05);1 250 ml组的总蛋白、血红蛋白、钠离子指标较营养支持前有所增加,差异有统计学意义(P<0.05);而1 500 ml匀浆膳补充组的所有其余7项指标均显著升高,差异有统计学意义(P<0.05)。结论 4种剂量自制新鲜食材匀浆膳均能在4周内改善患者钠离子水平并能有效控制患者血糖,同时均具有较好的胃肠道适应性。1 500 ml(约1 875 kcal)自制新鲜食材匀浆膳对重症脑卒中合并2型糖尿病患者的营养状况改善具有显著效果,值得临床推广和应用。Objective To investigate the nutritional support effects of different doses of fresh food homogenate meal on severe cerebral stroke complicated by type 2 diabetes mellitus (DM). Methods A total of 88 patients with severe cerebral stroke complicated by type 2 DM who were hospitalized in the department of rehabilitation of our hospital from January 2017 to March 2018 were randomly divided into four groups, and the patients in the four groups were given fresh food homogenate meal 750ml (about 818kcal), 1,000ml (about 1,108kcal),1,250ml (about 1,368kcal) and 1,500ml (about 1,875kcal), respectively, with a treatment course of 4 weeks.The canges of blood glucose indexes and the other biochemical parameters were observed before and after nutritional support by self-control method. Results After 4-week nutritional support intervention, the levels of fasting blood glucose and 2h postprandial blood glucose in the four groups were significantly decreased,as compared with those before nutritional support ( P <0.05).The levels of total protein and hemoglobin in 750ml group were significantly lower than those before nutritional support,however,the sodium ion levels were significantly higher than those before nutritional support ( P <0.05).The sodium ion levels in 1000ml group were significantly higher than those before nutritional support ( P <0.05).After nutritional support the levels of total protein, hemoglobin, sodium ion in 1250ml were significantly increased,as compared with those before nutritional support ( P <0.05).Moreover the levels of the other 7 indexes in 1500ml group were significantly increased after nutritional support ( P <0.05). Conclusion The four doses of fresh food homogenate meal can improve the sodium ion levels and effectively control the blood glucose of patients with severe cerebral stroke complicated by type 2 DM within 4 weeks, which have good gastrointestinal adaptability,in which, the 1,500ml (about 1,875kcal) homemade fresh food homogenate meals have significant effects in improving nutritional
