摘 要:目的探讨宫颈病变患者高危型HPV感染情况,以及血清和宫颈局部免疫因子的水平.方法选择温州医科大学附属第二医院和西安交通大学第二附属医院2014-2017年就诊的宫颈病变患者350例,其中高危型HPV感染者200例为高危感染组,其他为非高危组.ELISA法和流式CBA法分别测定患者血清和宫颈局部IL-2、TNF-α、IFNα水平,RT-PCR法测定高危型HPV载量.比较两组患者免疫因子水平,分析其与HPVDNA载量相关性.结果与非高危型感染组相比,高危感染组血清和宫颈局部IL-2水平较低(t=-32.861、-9.891,P<0.05),TNF-α和IFNα水平较高(TNF-α:t=35.872、5.037;IFNα:t=36.763、10.416,P均<0.05).高危型HPVDNA载量与血清和宫颈局部IL-2水平呈负相关(r=-0.513、-0.528,P<0.05),与血清和宫颈局部TNF-α、IFNα水平呈正相关(TNF-α:r=0.497、0.503;IFNα:r=0.554、0.571,P<0.05).结论超半数宫颈病变患者感染高危型HPV,宫颈高危型HPV感染可引起免疫紊乱.Objective To investigate the high-risk (HPV HR-HPV) infection in patients with cervical lesions,and to analyze serum and cervical part levels of IL-2,TNF-α and IFNα.Methods A total of 350 patients with cervical lesions were selected in the Second Affiliated Hospital of Wenzhou Medical University and the Second Affiliated Hospital of Xi'an Jiao Tong University from 2014 to 2017,including 200 patients with HR-HPV infection.The serum and cervical part levels of IL-2,TNF-α and IFNα were determined by ELISA and flow cytometry,respectively.The HR-HPV viral load was determined by reverse transcription-polymerase chain reaction (RT-PCR).Levels of immune factors were compared between the HR-HPV group and non-HR-HPV group,and further the relationship with HPV DNA was analyzed.Results The serum and cervical part levels of IL-2 in HR-HPV group were lower than those in non-HR-HPV group (t=-32.861,-9.891,P<0.05),and levels of TNF-α and IFNα were much higher (TNF-α:t=35.872,5.037;IFNα:t=36.763,10.416,P<0.05).The HR-HPV DNA load was negatively correlated with serum and cervical part levels of IL-2 (r=-0.513,-0.528,P<0.05),and positively correlated with serum and cervical part levels of TNF-α and IFNα(TNF-α:r=0.497,0.503;IFNα:r=0.554,0.571,P<0.05).Conclusions More than half of the patients with cervical lesions are infected with HR-HPV.The HR-HPV infection in the cervix can cause immune disorder.
