胃癌组织芯片XAF1和XIAP表达临床意义研究

摘　　要：目的细胞凋亡调控失衡在肿瘤发生发展中具有重要意义。本研究应用组织芯片技术,探讨胃癌组织中XAF1和XIAP等凋亡相关蛋白表达与胃癌临床病理特征和预后的相关性。方法将南通大学第二附属医院病理科存档的100例胃癌手术标本和30例正常胃粘膜标本制成组织芯片,用免疫组化SP法检测100例胃癌和30例正常胃粘膜组织XAF1和XIAP表达情况,用TUNEL法进行细胞凋亡指数检测。对随访资料完整的47例胃癌患者作生存分析。结果胃癌组织XAF1阳性率为62%,对照组胃黏膜组织无表达(χ~2=6.788,P=0.009);XAF1表达与胃癌TNM分期(P=0.019)和淋巴结转移(P=0.016)有关联,而与血管/神经侵犯(BV/N,χ~2=2.376,P=0.123)和组织学分化(χ~2=2.131,P=0.144)无关联。胃癌组织XIAP阳性率为31%,对照组胃黏膜组织无表达(χ~2=4.667,P=0.031);XIAP表达与胃癌组织学分化有关联(χ~2=6.629,P=0.010),与淋巴结转移(χ~2=0.049,P=0.824)、BV/N(χ~2=0.404,P=0.525)和TNM分期(P=0.936)无关联。胃癌组织XAF1与XIAP表达负相关(r=-0.366,P<0.001)。XAF1阳性组细胞凋亡指数高于阴性组(χ~2=6.79,P=0.009),而XIAP阳性组凋亡指数低于阴性组,χ~2=4.66,P=0.031。随访资料完整的47例患者中,单因素Cox分析显示,BV/N(χ~2=4.12,P=0.042)和TNM分期(χ~2=19.26,P<0.001)及XAF1表达(χ~2=0.030,P=0.030)与预后有关联,而XIAP蛋白表达(χ~2=0.68,P=0.408)及组织学分化(χ~2=0.19,P=0.660)与患者预后无关联。多因素Cox分析显示,仅TNM分期具有独立预后意义,P=0.001。结论 XAF1和XIAP均参与胃癌生物学进程,XAF1促进细胞凋亡,XIAP抑制细胞凋亡,XAF1与XIAP表达负向关联。XAF1表达与胃癌临床病理特征有关联,疑似为胃癌有利的预后指标,但不具有独立的预后意义。OBJECTIVE Unbalance of apoptosis regulation plays an important role in tumor initiation and progression.This study was to research the role of XAF1 and XIAP in apoptosis of gastric cancer,and its interrelation with the clinicopathologic features as well as the prognosis of gastric carcinomas using tissue microarray.METHODS The tissue microarrays were composed of these from 100 cases of gastric carcinomas and 30 normal stomach tissues of control subject from the data in pathological department of our hospital.The diameter of each specimen on tissue chip was 2.0 mm.The expressions of XAF1,XIAP were investigated immunohistochemically on these tissue chips.The carcinoma cell apoptosis index was explored by TUNEL method.Of the 100 cases of gastric carcinomas,47 cases were followed up from fourteen months to 13 years,and the prognosis was analyzed.RESULTS Immunostain analysis indicated that 62 cases(62%)was XAF1 positive in gastric cancer tissue chips(χ^2=6.788,P=0.009).The expression of XAF1 was associated with TNM stages(P=0.019)and lymph node involvement(P=0.016),but unrelated with invading of vessel & nerve(χ^2=2.376,P=0.123)and histological grades(χ^2=2.131,P=0.144).In contrast,XAF1 was non expression in control tissues.The expression of XIAP was positive in 31 cases in this series(31%,χ^2=4.667,P=0.031).XIAP expression was significantly related with histological grades(χ^2=6.629,P=0.010)but unrelated with lymph node involvement(χ^2=0.049,P=0.824),invading of vessel & nerve(χ^2=0.404,P=0.525)and TNM stages(P=0.936).The apoptosis index was higher in XAF1(+)group(3.53±1.16)than in XAF1(-)group(2.95±0.82)(χ^2=6.79,P=0.009).The apoptosis index was lower in XIAP(+)group(2.91±0.79)than that(3.62±1.17)in XIAP(-)group(χ^2=4.66,P=0.031).In the followed up data,univariant analysis showed that the survival rate was related to invading of vessel & nerve(χ^2=4.12,P=0.042),TNM stages(χ^2=19.26,P<0.001),and expression of XAF1(χ^2=0.030,P=0.030).Expression of XIAP(χ^2=0.68,P=0.408)and histological grades(χ^2=0.1
