两种不同机械止血法治疗急性消化性溃疡出血的比较

摘　　要：目的比较两种不同机械止血法治疗急性消化性溃疡出血的效果及安全性。方法急性消化性溃疡大出血12例为研究组,使用OTSC止血; 43例为对照组,使用传统金属夹止血。统计分析两组的性别、年龄、Hb、Forrest分级数、操作时间、止血率、7天内再出血率、并发症率等指标。结果研究组和对照组组之间性别构成、年龄、Hb、Forrest分级构成等指标的差异无统计学意义。研究组操作时间短(18. 82±3. 84&30. 79±4. 72) min,P <0. 05,止血率高(90. 91%&60. 47%),7天内再出血率低(0%&32. 56%%,P<0. 05)。两组并发症发生率相当(0%)。结论两种不同机械止血法中OTSC治疗急性消化性溃疡大出血操作简便、有效、安全,且更能减少再出血的发生。Objective To compare the efficacy and safety of two different mechanical hemostasis in the treatment of acute peptic ulcer bleeding.Methods Twelve patients with acute peptic ulcer hemorrhage in the study group were treated with OTSC to stop bleeding.Forty-three patients in the control group were treated with traditional metal clips to stop bleeding.The gender,age,Hb,Forrest classification,operation time,hemostasis rate,rebleeding rate within 7 days and complication rate of the two groups were statistically analyzed.Results There were no significant differences in gender composition,age,Hb and Forrest classification between the study group and the control group.The operation time of the study group was shorter(18.82±3.84 & 30.79±4.72 minutes,P <0.05),the hemostasis rate of the study group was higher(90.91%& 60.47%),and the rebleeding rate within 7 days of the study group was lower(0%& 32.56%,P <0.05).The incidence of complications was comparable in both groups(0%).Conclusion Of the two different mechanical hemostasis procedures in the treatment of acute peptic ulcer bleeding,OTSC is simple,effective,safe,and can reduce the occurrence of rebleeding.
