宁夏包虫病历调查和防治项目实施后治疗负担分析

摘　　要：目的通过宁夏病历调查,评估包虫病发病情况以及包虫防治项目实施对全区患者疾病治疗负担的影响效果。方法对宁夏18所医院的包虫病历信息(2002-2013年)进行回顾性调查,整理资料并进行数据分析。结果共收集2333份包虫病例,宁夏医科大学总医院收治包虫病患者646例,人均缴费为14612元/例。接受手术治疗的患者共1476例,占患者总例数的63.27%。中卫市医院手术治疗患者占当地患者总例数的91.35%,海原县医院占当地患者总数38.27%。宁夏本地患者中,石嘴山市中平罗县患病例数最高(26.50■),银川市中永宁县患病率最高(36.70■),中卫市中海原县患病率最高(69.40■),吴忠市中同心县患病率最高(89.30■),固原市中彭阳县患病率最高(57.90■)。患者中农村居住地者为主要人群,占81%。包虫病患者30岁～组患病率为4.97■;其中0岁～、80岁以上年龄组中男性患病率高于女性,50岁～年龄组中女性患病率高于男性;自2007年免费手术治疗政策全面实施后,各地区缴费人数比例有不同程度的下降。结论包虫病仍高发于宁夏南部,中央补助防治项目可有效减轻患者的治疗负担。Objective To evaluate the effect of the Ningxia echinococcosis prevention and control project through retrospective study of the clinical records of human echinococcosis and their treatment cost.MethodsThe study was conducted by collecting the clinical records of echinococcosis patients duration 2002-2013 from hospitals within Ningxia Hui Autonomous Region.ResultsA total of 2,333 cases were collected,and 646 cases were from the General Hospital of Ningxia Medical University.The per capita contribution was 14612 yuan.There were1476 patients underwent surgery,accounting for 63.27%.The surgical treatment of Zhongwei City Hospital accounted for 91.35%,and Haiyuan County Hospital accounted for38.27%.The number of cases in Pingluo County was the highest(26.50■)in Shizuishan,the prevalence rate of Yongning County was the highest(36.70■)in Yinchuan City,and the prevalence rate of Haiyuan County was the highest(69.40■)in Zhongwei.Wuzhong City has the highest prevalence rate in Tongxin County(89.30■),and Pengyuan County was the highest prevalence rate(57.90■)in Guyuan.Among the patients,the rural was the main population,accounting for 81%.The prevalence rate of echinococcosis patients in the 30age group was 4.97■;among them,the prevalence rate of males in the 0-and 80-age group was higher than that in females,and the prevalence rate of women in the 50-age group higher than male;Since the implementation of the free surgical treatment policy in 2007,the proportion of people paying in various regions has declined.ConclusionThe retrospective(2002-2013)study revealed the hyper-endemic areas of echinococcosis were still located in southern parts of Ningxia.The project of transmission prevention and patient-management launched by the Central Government of China has paved the road for reducing the disease burden of human echinococcosis,and control effectively.
