益气升白汤治疗非小细胞肺癌化疗后白细胞减少症的临床疗效

摘　　要：目的:探究益气升白汤对于非小细胞肺癌(non-small cell lung cancer,NSCLC)患者化疗后急性白细胞减少症的治疗效果。方法:将82例入组患者(全血白细胞计数≤3.0×10~9/L)随机分为研究组(n=41)和对照组(n=41)。所有患者均给予维生素B_4、鲨肝醇及利血生治疗,对照组在此基础上给予粒细胞刺激因子治疗,研究组则给予益气升白汤剂治疗,治疗一个月后比较两组病例外周血白细胞减少程度、生活质量以及中医症候评分。结果:两组总有效率无显著差异(80.5%vs 78.0%,P=0.785)。对照组平均显效时间短于研究组,差异有统计学意义(4.2±1.3天vs 8.3±1.6天,P=0.000)。研究组维持白细胞正常以上的平均时间明显高于对照组,差异有统计学意义(15.4±1.5天vs 10.2±1.7天,P=0.000)。治疗后研究组患者KPS评分显著高于对照组(P=0.000)。治疗后,研究组实症评分明显低于对照组(P=0.027)。结论:益气升白汤治疗非小细胞肺癌化疗后的白细胞减少症与粒细胞刺激因子总有效率相当,且明显改善患者症状。Objective:To investigate the therapeutic effect of Yiqi Shengbai Decoction on acute leukopenia after chemotherapy in patients with non-small cell lung cancer(NSCLC).Methods:Eighty-two patients(WBC count ≤ 3.0 × 109/L) were randomly divided into study group(n=41) and control group(n=41).All patients were treated with vitamin B4,shark alcohol and reserpine.The control group was treated with granulocyte-stimulating factor,while the study group was treated with Yiqi Shengbai Decoction on the basic treatment.After one month of treatment,the peripheral blood grade of leukopenia,quality of life and TCM syndrome score were compared and analyzed.Results:There was no significant difference of total effective rate between the two groups(80.5% vs 78.0%,P=0.785).The difference was statistically significant for effective time(4.2±1.3 days vs 8.3±1.6 days,P=0.000) in the control group compared with the study group.The mean time to maintain normal white blood cells of the study group was significantly higher than the control group,and the difference was statistically significant(15.4±1.5 days vs 10. 2 ± 1. 7 days,P = 0. 000). After treatment,the KPS score of the study group was significantly higher than that of the control group( P = 0. 000). After treatment,the real symptoms score of the study group was significantly lower than that of the control group( P = 0. 027). Conclusion: The total effective rate of Yiqi Shengbai Decoction for the treatment of leukemia after chemotherapy for patients with non-small cell lung cancer is similar to granulocyte-stimulating factor,and could significantly improve the symptoms of patients.
