摘　　要：目的:探讨过氧化物酶4(peroxiredoxin 4,Prx4)和NF-κB在外阴硬化性苔藓(vulvar lichen sclerosis,VLS)和外阴鳞癌(vulvar squarnous cell carcinoma,VSCC)中的表达及意义。方法:采用免疫组化方法检测Prx4和NF-κB在VLS和VSCC及外阴正常组织中的表达情况,并结合临床病理资料进行分析。结果:Prx4在VLS的表达明显高于外阴正常皮肤,两者有统计学差异(P<0.05);在VSCC中的表达明显高于外阴正常皮肤(P<0.05)。在VSCCⅢ-Ⅳ期中表达高于Ⅰ-Ⅱ期(P<0.05);有淋巴结转移的表达高于无淋巴结转移(P<0.05),中分化的表达高于高分化(P<0.05)。NF-κB在VLS中的表达明显低于外阴正常皮肤,两者有统计学差异(P<0.05);NF-κB在VSCC中的表达明显高于外阴正常皮肤(P<0.05),在VSCCⅢ-Ⅳ期中表达高于Ⅰ-Ⅱ期(P<0.05);有淋巴结转移的表达高于无淋巴结转移(P<0.05),中分化的表达高于高分化(P<0.05)。Spearman相关性分析结果表明:在VLS中Prx4和NF-κB的表达呈负相关;在VSCC中,两者表达呈正相关。结论:Prx4和NF-κB在VSCC中高表达;NF-κB在VLS中低表达。Objective:To investigate the expression of Prx4 and NF-κB proteins in vulvar lichen sclerosis(VLS) and vulvar squarnous cell carcinoma(VSCC).Methods:We used immunohistochemical method to detect the expression of peroxiredoxin 4(Prx4) and NF-κB proteins in VLS and VSCC as well as normal vulva skin,analysed the relationship of Prx4 and NF-κB with clinical and pathological data.Results:The expression of Prx4 in VLS and VSCC was significantly higher than that in normal vulva skin(P<0.05). Higher expression Prx4 was observed in stage Ⅲ-Ⅳ of VSCC(P<0.05),with lymph node metastasis was higher than that without lymph node metastasis(P<0.05) and the moderate differentiation was higher than that with well differentitation of VSCC(P<0.05).The expression of NF-κB in VLS was significantly lower than that in normal vulva skin,on the contrary NF-κB was higher expression in VSCC(P<0.05).NF-κB expression in stage Ⅲ-Ⅳ was higher than that of stage Ⅰ-Ⅱ(P<0.05),with lymph node metastasis was higher than that without lymph node metastasis(P<0.05),with moderate differentiation was higher than that with well differentiation(P<0.05).The Spearman correlation analysis showed that Prx4 and NF-κB expression was negatively correlated in VLS,and positively correlated in VSCC.Conclusion:The higher expression of Prx4 and NF-κB proteins promotes VSCC development.The lower expression of NF-κB protein is associated with the occurrence of VLS.
