帕金森综合征合并不宁腿综合征的精神行为特点

摘　　要：目的观察帕金森综合征合并不宁腿综合征(RLS)的精神行为特点及低频重复电刺激联合药物治疗的疗效。方法 30例患者分别接受传统药物治疗以及传统药物联合低频重复经颅磁刺激治疗,采用RLS病情严重程度评定量表(RLSRS)、统一帕金森病评分量表(UPDRS)、匹兹堡睡眠质量指数量表(PSQI)、汉密尔顿焦虑量表(Hamilton anxiety scale,HAMA)、汉密尔顿抑郁量表(Hamilton depression scale,HAMD)对两组患者进行评价。结果两组患者治疗后第14天、出院后两个月的RLSRS、UPDRS、PSQI、HAMA、HAMD评分明显低于治疗前,差异有统计学意义(P<0.05);rTMS联合药物组治疗后第14天,出院后两个月的RLSRS、UPDRS、PSQI、HAMA、HAMD评分明显低于对照组,差异有统计学意义(P<0.05)。且仅有1例患者有轻微不良反应。结论帕金森综合征合并不宁腿综合征的患者焦虑及抑郁现象更明显,药物联合低频重复经颅磁刺激能改善患者的运动能力、精神状态以及睡眠质量。Objective To observe the mental behavior characteristics of Parkinson's syndrome with restless legs syndrome and the efficacy of low-frequency repeated electrical stimulation combined with drug therapy.Methods A total of 30 Parkinson's syndrome patients with RLS were randomly divided into control group and rTMS/drugs group (15 cases in each group).The RLS Rating Scale (RLSRS),Unified Parkinson Disease Rating Scale (UPDRS) and Pittsburgh Sleep Quality Index (PSQI),Hamilton anxiety scale (HAMA),Hamilton depression scale (HAMD) were evaluated.Results RLSRS,UPDRS,PSQI,HAMA,HAMD scores on the 14th day,2 months after treatment were significantly lower than before therapy (P< 0.05).RLSRS,UPDRS,PSQI,HAMA,HAMD scores in control group on the 14th day,2 months after treatment were significantly higher than rTMS/drugs group.Only one patient had mild adverse effect.Conclusion Anxiety and depression are more obvious in patients with Parkinson's syndrome combined with restless legs syndrome.Drug combined with low-frequency repetitive transcranial magnetic stimulation can improve exercise ability,mental state and sleep quality.
