摘  要:目的探讨微小RNA-92a(miR-92a)在前列腺癌组织中的表达水平及其临床意义。方法收集本院2013年12月至2016年12月收治的前列腺癌患者的癌组织97例和癌旁组织标本62例,采用实时荧光定量PCR(QPCR)检测癌组织和癌旁组织样本中miR-92a的表达水平,分析癌组织miR-92a水平与临床病理学参数(年龄、临床分期、淋巴结转移、Gleason评分、术前PSA水平及远处转移)的关系,采用受试者工作特征曲线(ROC)评价组织中miR-92a的表达水平在前列腺癌早期诊断及区分不同临床病理参数的效能。结果前列腺癌组织中的miR-92a水平为3.497±1.993,高于癌旁组织的1.165±0.623,差异有统计学意义(P<0.05);ROC曲线分析显示miR-92a区分前列腺癌与正常前列腺组织的曲线下面积(AUC)为0.8832(95%CI:0.8327~0.9336,P<0.001),当miR-92a的界值取2.337时,灵敏度为72.16%、特异度为93.55%。miR-92a的表达水平与年龄、术前PSA水平及远处转移均无关(P>0.05),而与临床分期、淋巴结转移和Gleason评分有关(P<0.05)。ROC曲线分析显示miR-92a区分临床分期、淋巴结转移及Gleason评分的效能较好,AUC分别为0.7378、0.8758和0.8292,差异均有统计学意义(P<0.05);当临界值取3.405、3.435和2.895时,miR-92a区分临床分期、淋巴结转移及Gleason评分的灵敏度分别为67.24%、89.29%和83.61%,特异度分别为79.49%、71.01%和75.00%。结论 miR-92a高表达于前列腺癌组织,其表达水平与临床分期、淋巴结转移和Gleason评分有关,对于前列腺癌的诊断有一定价值,可以作为评估前列腺癌恶性程度的一个诊断指标。Objective To investigate the expression level and clinical significance of miRNA-92 a(miR-92 a) in the prostate cancer tissues. Methods From December 2013 to December 2016, 97 cases of prostate cancer and 62 specimens from para cancer tissue were collected in our hospital. The expression of miR-92 a in cancer tissue and para cancerous tissue samples was detected by real-time fluorescence quantitative PCR(QPCR). The relationship between miR-92 a level and the clinicopathological parameters(age, clinical stage, lymph node metastasis, Gleason score, preoperative PSA level and distant metastasis) was analyzed. The efficacy of miR-92 a in the early diagnosis of prostate cancer and the differentiation of different clinicopathological parameters was evaluated by the receiver operating characteristic(ROC) curves. Results The level of miR-92 a in the prostate cancer tissues was 3.497±1.993, higher than 1.165±0.623 of the paracancerous tissue, and the difference was statistically significant(P<0.05). The ROC curve analysis showed that the area under the curve(AUC) of miR-92 a was 0.8832(95%CI: 0.8327-0.9336, P<0.001) to differentiate prostate cancer and normal prostate tissue. When the boundary value of miR-92 a was 2.337, the sensitivity and specificity were 72.16% and 93.55%, respectively. The expression level of miR-92 a was not related to age, preoperative PSA level and distant metastasis(P>0.05), which was related to clinical stage, lymph node metastasis and Gleason score(P<0.05). ROC curve analysis showed that miR-92 a exhibited better performance to differentiate clinical stage, lymph node metastasis and Gleason score with the AUCs of 0.7378, 0.8758 and 0.8292(P<0.05). When the critical value of miR-92 a was 3.405, 3.435 and 2.895 for clinical stage, lymph node metastasis and Gleason score, the sensitivity was 67.24% 89.29% and 83.61% and the specificity was 79.49%, 71.01% and 75.00%.
