摘 要:目的分析与研究甲状腺浸润性微小癌的超声征象与病理分级的相关性。方法收集经病理检查证实行切除活检术的甲状腺浸润性微小癌患者108例,记录超声征象、病理分级并进行相关性分析。结果在108例患者中,病理分级为Ⅰ型68例,Ⅱ型25例,Ⅲ型15例。非包膜内型微小癌的形态、边界、纵横比、内部回声、钙化等与包膜内型微小癌对比差异有统计学意义(P <0. 05)。非包膜内型微小癌的超声血流成像评分显著高于包膜内型微小癌(P <0. 05)。相关分析显示病理分级与临床分期、分化类型、超声形态、边界、纵横比、内部回声、钙化征象与超声血流成像评分均呈显著相关性(P <0. 05)。结论甲状腺浸润性微小癌的超声征象与病理分级有显著相关性,非包膜内型微小癌多表现为形态不规则、边界不清晰、内部低回声、钙化与血流丰富等。Objective To analysis and study the correlation between ultrasound signs and pathological grades of thyroid infiltrative microcarcinoma. Methods A total of 108 cases of invasive micro carcinoma of thyroid by pathological examination were collected,and the ultrasonography and pathological grading were recorded and were given correlation analysis. Results Te pathological grade were 68 cases in type Ⅰ,25 cases in typeⅡ,and 15 cases in type Ⅲ in the 108 cases. The morphology,boundary,aspect ratio,internal echo,calcification,etc. of non-enveloped microcarcinoma were significantly different from those of intracapsular microcarcinoma( P < 0. 05). The ultrasound blood flow imaging score of non-encapsulated microcarcinoma were significantly higher than that of intracapsular microcarcinoma( P < 0. 05). Correlation analysis showed there were significant correlation between pathological grade and clinical stage,differentiation type,ultrasound morphology,boundary,aspect ratio,internal echo,calcification signs and ultrasound blood flow imaging scores( P < 0. 05). Conclusion The ultrasonographic signs of thyroid infiltrative microcarcinoma are significantly correlated with pathological grade. The non-enveloped microcarcinomas are characterized by irregular shape,unclear boundary,low internal echo,calcification and abundant blood flow.
