自体角膜缘干细胞结合结膜移植联合翼状胬肉切除术治疗原发性翼状胬肉的临床效果分析

摘　　要：目的探讨自体角膜缘干细胞结合结膜移植联合翼状胬肉切除术治疗原发性翼状胬肉的效果。方法收集大同朝聚安康眼科医院2017年1-12月收治的93例(95眼)原发性翼状胬肉,随机分为观察组47例(48眼,行翼状胬肉切除联合自体角膜缘干细胞+结膜移植术)、对照组46例(47眼,行翼状胬肉切除术),患者术后1 d、3 d、1周、2周、12个月时复诊,记录患者术后角膜上皮恢复时间及治疗效果和复发情况。结果角膜上皮修复时间:观察组(4.6±0.4)d,对照组(7.8±1.1)d,差异具有统计学意义(P<0.05)。观察组治愈47眼、复发1眼;对照组治愈39眼、复发8眼,观察组治愈率高于对照组,差异具有统计学意义(P<0.05)。结论自体角膜缘干细胞+结膜移植联合翼状胬肉切除术有利于术后角膜上皮恢复,复发率低,值得推广。Objective To investigate the efficacy of autologous limbal stem cell conjunctival transplantation combined with pterygium excision in the treatment of primary pterygium.Methods Ninety-three patients(95 eyes)with primary pterygium admitted to our hospital from January to December 2017 were randomly divided into observation group(n=47 patients,48 eyes)with pterygium excision combined with autologous limbal stem cells plus conjunctiva transplantation,and control group(n=46 patients,47 eyes)underwent pterygium resection.The patients were followed up for 1 day,3 days,1 week,2 weeks,and 12 months.The recovery time of corneal epithelium and the recurrence rate of pterygium were recorded in all patients.Results The corneal epithelial repair time was(4.6±0.4)d in the observation group and(7.8±1.1)d in the control group.The recurrence rate was 2.1%(one eye recurrence)in the observation group,which was significantly lower than 20.51%in the control group(8 eyes recurrence)(P<0.05).Conclusion Autologous limbal stem cell plus conjunctival transplantation combined with pterygium resection is beneficial to the recovery of postoperative corneal epithelium,and the recurrence rate is low,which is worthy of promotion.
