摘  要:目的探讨喉癌患者原发病灶及分子切缘肿瘤标志物表达与肿瘤复发的关系。方法将238例喉癌患者依据是否复发分为复发组(46例)与未复发组(192例)。采用免疫组化技术分别检测两组原发灶、分子切缘肿瘤标志物阳性表达情况,比较两组原发灶、分子切缘肿瘤标志物阳性表达率,分析原发灶、分子切缘肿瘤标志物阳性表达与喉癌复发的相关性。结果(1)复发组原发灶人体抑癌基因53、G1/S-特异性周期蛋白-D1阳性表达率显著高于未复发组(P<0.01),原发灶人体抑癌基因27阳性表达率显著低于未复发组(P<0.01);(2)复发组分子切缘真核起始因子4E、人体抑癌基因53、G1/S-特异性周期蛋白-D1阳性表达率显著高于未复发组(P<0.01),分子切缘人体抑癌基因27阳性表达率显著低于未复发组(P<0.01);(3)原发灶人体抑癌基因53、G1/S-特异性周期蛋白-D1阳性表达与喉癌复发呈显著正相关(P<0.01),原发灶人体抑癌基因27阳性表达与喉癌呈显著负相关(P<0.05);(4)分子切缘真核起始因子4E、人体抑癌基因53、G1/S-特异性周期蛋白-D1阳性表达与喉癌复发呈显著正相关(P<0.01),分子切缘人体抑癌基因27阳性表达与喉癌复发呈显著负相关(P<0.01)。结论喉癌患者原发灶、分子切缘肿瘤标志物人体抑癌基因53、人体抑癌基因27、G1/S-特异性周期蛋白-D1及分子切缘真核起始因子4E表达与喉癌复发密切相关,检测上述指标的变化对预测复发及制定防治措施具有重要意义。Objective To investigate the relationship between primar lesions and molecular margin tumor markers and tumor recurrence in patients with laryngeal cancer.Methods A total of 238 patients with laryngeal cancer divided into recurrent group(n=46)and non-recurrent group(n=192)according to whether they were relapsed.Immunohistochemical technique was used to detect the positive expression of tumor markers in the two groups of primary tumors and molecular margins.The positive expression rate of the primary tumor and molecular margin tumor markers was compared.Analyze the correlation between the positive expression of primary tumor and molecular margin tumor markers and recurrence of laryngeal cancer.Results(1)The positive expression rate of P53 and CyclinD1 in the primary tumor of the recurrent group was higher than that in the non-recurrent group(P<0.01).The positive expression rate of P27 in the primary tumor was lower than that in the non-recurrent group(P<0.01).(2)The positive expression rate of eIF4E,P53 and CyclinD1 in the recurrence group was higher than that in the non-recurrence group(P<0.01),and the positive expression rate of P27 was lower than that in the non-recurrent group(P<0.01).(3)The positive expression of P53 and CyclinD1 in primary tumor was positively correlated with recurrence of laryngeal carcinoma(P<0.01).The positive expression of P27 in primary tumor was negatively correlated with laryngeal carcinoma(P<0.05).(4)The positive expression of eIF4E,P53 and CyclinD1 in the molecular margin was positively correlated with the recurrence of laryngeal carcinoma(P<0.01).The positive expression of P27 in the molecular margin was negatively correlated with the recurrence of laryngeal carcinoma(P<0.01).Conclusion In patients with laryngeal cancer,primary lesions and P53,P27,CyclinD1,eIF4E expressions are closely correlated with recurrence of laryngeal carcinoma.The detection of changes in the above indicators is of great significance for predicting recurrence and developing prevention and treatment measures.
