鼻内镜下经口腺样体不同切除术疗效对照观察

摘　　要：目的探讨鼻内镜下腺样体切吸器切除术与低温等离子切除术治疗腺样体肥大的效果。方法将腺样体肥大的333例患儿中行鼻内镜下腺样体切吸器切除的156例患儿设为切吸组,低温等离子切除的177例患儿设为低温等离子组。统计两组手术时间、术中出血量、止血时间、临床疗效及复发率等。结果切吸组手术时间、止血时间显著长于低温等离子组(P<0.01),术中出血量显著多于低温等离子组(P<0.01),腺样体残留率、鼻腔粘连率及复发率显著高于低温等离子组(P<0.05或0.01)。两组临床疗效比较差异无统计学意义(P>0.05)。结论鼻内镜下切吸器切除术和低温等离子切除术治疗腺样体肥大均有效,但低温等离子切除腺样体手术时间短、创面出血少,止血方便,并发症发生率及复发率更低。Objective To investigate the effect of nasal endoscopic adenoidectomy and low temperature plasma resection for adenoid hypertrophy.Methods A total of 333 children with adenoid hypertrophy were treated with two different surgical methods.Among them,156 cases treated by endoscopic adenoidectomy were seen as the adenoidectomy group,177 cases treated by low temperature plasma resection were seen as the low temperature plasma group.The differences of operation duration,intraoperative bleeding volume,duration of hemostasis and recurrence rate of the two groups were compared.Results The operation time and hemostasis time of the adenoidectomy group were significantly longer than those of the low temperature plasma group(P<0.01),and the intraoperative blood loss was significantly higher than that of the low temperature plasma group(P<0.01).Adenoid residual rate,nasal adhesion rate and recurrence rate were significantly higher than those in low temperature plasma group(P<0.05 or 0.01).There was no significant difference in clinical efficacy between the two groups(P>0.05).Conclusion Both nasal endoscopic adenoidectomy and low temperature plasma resection have good therapeutic effect,but low-temperature plasma adenoid surgery has a short operation time,less wound bleeding,convenient hemostasis,lower complication rate and recurrence rate.
