摘　　要：目的:探讨原发性肝淋巴瘤(primary hepatic lymphoma,PHL)的多种影像表现,以提高对该病的诊断水平。方法:收集我院2008-03-01～2017-12-31经穿刺活检或手术切除病理证实的21例PHL患者的临床资料,回顾性分析其多种影像表现。结果:21例患者病灶多位于肝右叶,肝左叶和尾状叶病灶相对较少;病灶呈圆形或类圆形,直径2.3～13.7 cm,平均5.6 cm。CT平扫示肝内低密度或稍低密度影,病灶边界清楚,密度均匀;增强扫描病灶边缘明显强化,病灶内部轻度强化,可见"血管漂浮征"。MRI平扫示病灶T1WI呈低信号,T2WI呈高信号,DWI病灶呈高信号,MRI增强病灶强化特征与CT相似。PET/CT检查示,所有病灶均可见放射性摄取异常浓聚,标准摄取值(standard uptake value,SUV)为4.4～19.3,平均12.6;延迟1 h PET/CT显像示,所有病灶的SUV值均不同程度升高。结论:PHL的多种影像检查具有不同的影像学特征,联合运用这些影像检查结果,可明显提高其诊断水平。Objective:To explore the imaging features of primary hepatic lymphoma(PHL),so as to improve the diagnostic level.Methods:Data of 21 patients with PHL confirmed by pathology in our hospital from March 2008 to December 2017 were collected and image manifestations was retrospectively analyzed.Results:The lesions of 21 patients were mostly located in the right lobe of the liver,with relatively few lesions in the left lobe and caudate lobe of the liver.The lesion was round or oval,the diameter was 2.3-13.7 cm,with an average diameter was 5.6 cm.CT plain scan showed low density or slightly low density shadow in the liver,the lesions had clear boundaries and uniform density.On contrast-enhanced scan,the edge of the lesion was markedly enhanced,the interior of the lesion was slightly enhanced,and "vascular floating sign" could be seen in the lesion.MRI plain scan showed that T1 WI showed low signal intensity,T2 WI showed high signal,and DWI lesion showed low signal.MRIenhanced focus features and CT resemble.PET/CT examination showed that all the lesions showed abnormal concentration of radioactive uptake.The standard uptake value(SUV) value was 4.4-19.3,with an average of 12.6.Delayed 1 hour PET/CT imaging showed that the SUV values of all lesions increased to varying degrees.Conclusion:Multiple imaging findings of PHL have different imaging characteristics.Combined application of multiple imaging examinations can significantly improve the diagnostic level.
