摘　　要：目的探讨替莫唑胺联合三维适形放疗在恶性胶质瘤术后的应用效果。方法将72例脑胶质瘤患者按照其入院后手术治疗的时间分为对照组与观察组,各36例。两组患者均实施手术切除,术后2周实施三维适形放射治疗,观察组在此基础上术后口服替莫唑胺。比较两组患者的临床疗效、治疗前后的生存质量评分、2年存活情况和治疗期间不良反应发生情况。结果观察组患者的治疗总有效率为61.11%,明显高于对照组的33.33%(P<0.05);治疗后,两组患者的生存质量评分均高于治疗前,且观察组高于对照组(P<0.05);观察组2年存活率显著高于对照组(P<0.05);治疗期间,两组患者的粒细胞减少、脱发、贫血及恶心呕吐总发生率比较,差异无统计学意义(P>0.05)。结论恶性胶质瘤术后采用替莫唑胺联合三维适形放疗可提高患者的疗效和术后生存质量,提高患者术后2年存活率。Objective To investigate the application effect of temozolomide combined with three dimensional conformal radiotherapy in the treatment of malignant glioma after operation. Methods A total of 72 patients with glioma were divided into control group and observation group according to the operation time after admission, with 36 cases in each group. Both groups underwent surgical resection and were given three dimensional conformal radiotherapy at 2 weeks after operation. On this basis, the observation group was given temozolomide oral after operation. The clinical effects, quality of life scores before and after treatment, 2-year survival and adverse reactions during treatment in the two groups were compared. Results The total effective rate of treatment in the observation group was 61.11%, which was significantly higher than 33.33% in the control group(P<0.05). After treatment, the quality of life scores in the two groups were higher than those before treatment,and those in the observation group were significantly higher than the control group(P<0.05). The 2-year survival rate of the observation group was significantly higher than that in the control group(P<0.05). There were no significant differences in the total incidences of granulocytopenia, alopecia, anemia and nausea and vomiting between the two groups(P>0.05). Conclusion Temozolomide combined with three dimensional conformal radiotherapy on malignant glioma after operation can improve the curative effect and postoperative quality of life, and improve patients’ 2-year survival rate after operation.
