摘 要:目的:观察和评价在大样本血液肿瘤患者人群中采用异甘草酸镁注射液预防抗肿瘤化疗相关性急性肝损伤的有效性和安全性。方法:对511例患者化疗开始前1d给予200mg的异甘草酸镁注射液静脉滴注,1次/d,连续用药至1个化疗疗程结束后,异甘草酸镁注射液用药≥7d。观察化疗前后肝脏生化指标的变化及输注异甘草酸镁的不良反应。结果:共入组1567例接受化疗的恶性血液病患者,纳入全分析集(FAS)分析1511例。化疗后ALT、AST、T-BIL、D-BIL、ALP及GGT较化疗前均无增高。化疗过程部分患者出现不同程度肝损伤,其中TBIL异常率最高,为15.19%。化疗前肝功能异常的患者,化疗后肝功能较前好转,化疗前后肝功能指标改善有统计学差异。患者在治疗前后肾功能指标、电解质、血压等比较,没有统计学差异。结论:异甘草酸镁注射液与化疗药物同时应用,能够防治血液肿瘤化疗药物相关性肝损伤,明显降低其发生率。Objective:To observe and evaluation the efficacy and safety of magnesium isoglycyrrhizinate injection to prevent liver injury induced by chemotherapy in hematological malignancies.Methods:The magnesium isoglycyrrhizinate injection was given 200mg per day before the chemotherapy,and after the end of chemotherapy courses.magnesium isoglycyrrhizinate injection was used for at least 7 days.The changes of liver biochemical indexes before and after chemotherapy and the adverse reactions of magnesium Isoglycyrrhizinate Injection were observed.Results:Among the 1567 cases of hematological malignancies received chemotherapy,1511 cases were included in the FAS analysis.After chemotherapy,the levels of ALT,AST,TBIL,DBIL,ALP and GGT were not increased compared with those before chemotherapy.Some patients suffered from different degrees of liver injury during chemotherapy.Among them,the most common abnormalities were TBIL up to 15.19%.The liver function of patients with abnormal liver function before chemotherapy got better.There was significant difference in the improvement of liver function before and after chemotherapy.There was no statistical difference in renal function index,electrolyte and blood pressure before and after treatment.Concusions:The application of magnesium isoglycyrrhizinate injection with chemotherapeutic drugs can prevent the liver injury of chemotherapeutic drugs.
