124颗外伤脱位恒牙的临床分析研究

摘　　要：目的探讨外伤脱位恒牙能获得良好远期疗效的临床关键因素。方法对2013年度1月—2018年5月来本院综合科急诊的68例患者共124颗外伤脱位恒牙进行了回顾性临床资料整理,就外伤牙发病情况、临床检查、治疗过程进行分类汇总,并同时进行了临床随访和临床疗效研究,对脱位牙治疗前和治疗完成后随访时的松动情况进行统计分析。结果随访到的110颗患牙经复位固定治疗后,77颗牙(70.0%)无明显松动,27颗牙(24.5%)I度松动,6颗牙(5.5%)有Ⅱ度松动。与复位治疗前相比,松动度有显著差异性(P<0.001),同时X线显示患牙中外伤脱位后进行过根管治疗的共有19颗牙(17.3%),而91颗外伤保留牙((82.7%)未行根管治疗。结论尽快复位制动为外伤脱位牙治疗的关键因素,而根管治疗则在牙髓发生异常时进行,可获得较好的保留和美学、咬合及发音等远期疗效。Objective To explore the key factors for expected results of luxation of traumatized permanent teeth.Methods 124 teeth of 68 emergency patients with luxation of injured teeth from January 2013 to May 2018 in department of general dentistry were enrolled in this study.The injured factors,clinical examinations,the procedures of treatment,and the results of followed up were analyzed.Results After replantation and fixation,all 110 teeth of the followed up,77 teeth(70.0%)have no various loosening,27 teeth(24.5%)have typeⅠloosening and 6 teeth(5.5%)have typeⅡloosening.There is significant difference compared to pretreatment(P<0.001).Besides,from X ray there are 19 teeth(17.3%)were under root canal therapy(RCT),and 91 teeth(82.7%)without RCT after injured.Conclusion Based on current study,replantation without various articulation,is the vital factors for the traumatized luxation tooth,with good function of aesthetics,occlusion and pronunciation in long run.As for root canal therapy,the treatment relies on the evaluation of tooth pulp of the injured tooth during followed up.
