摘 要:目的:探讨口腔扁平苔藓(OLP)患者血清中神经生长因子(NGF)和P物质(SP)的表达及意义。方法:流式细胞术及散射比浊法分析30例OLP患者和20例健康对照组外周血淋巴细胞亚群(CD3+、CD4+、CD8+、CD19+、CD16+56+)和体液免疫指标(IgG、IgA、IgM、C3、C4)的表达。酶联免疫吸附试验(ELISA)检测OLP患者血清NGF和SP的表达并分析其与OLP免疫功能的相关性。结果:OLP患者CD3+、CD4+、CD8+、C3、C4低于正常值(P<0.05),CD19+、CD4+/CD8+及IgM高于正常值(P<0.05)。OLP组的NGF和SP均高于对照组(P<0.05),糜烂型OLP高于非糜烂型(P<0.05)。OLP组NGF和SP表达呈正相关且二者均与OLP体征计分正相关(P<0.05),与CD19+、IgA含量呈正相关(P<0.05)。结论:NGF和SP的高表达可能在OLP中参与了神经系统对免疫应答的调节作用,二者共同参与了OLP炎症发展。Objective: To investigate the expression and significance of nerve growth factor(NGF) and substance P(SP) in serum of patients with oral lichen planus(OLP). Methods: The peripheral blood lymphocyte subsets(CD3+, CD4+, CD8+, CD19+, CD16+56+) and humoral immunity(IgG, IgA, IgM, C3, C4) in 30 patients with OLP were detected by flow cytometry and nephelometry assays respectively, NGF and SP contents in serum were detected by ELISA. The correlations between the levels of NGF, SP, immune status and clinical characteristics of OLP patients were analyzed respectively. 20 healthy subjects were served as the controls. Results: CD3+, CD4+, CD8+, C3 and C4 in OLP patients were lower than the normal values(P<0.05), whereas the expression of CD19+, CD4+/CD8+ and IgM was higher than the normal values(P<0.05). NGF and SP levels showed significant upregulation in OLP patients compared with control group(P<0.05), and in the OLP patients of erosive type was higher than those of non-erosive type(P<0.05). Meanwhile, positive correlations existed between NGF and SP in the OLP serum(P<0.05), both of them were positively correlated with OLP clinical characteristics(P<0.05), CD19+(P<0.05) and IgA(P<0.05), respectively. Conclusion: Over expression of NGF and SP may be involved in the communication of nervous-immune system of OLP and promote the development of OLP.
