腹腔镜与开腹手术治疗结直肠癌的疗效比较

摘　　要：为观察比较腹腔镜与常规开腹手术治疗结直肠癌的临床效果,将行手术治疗的90例结直肠癌患者随机分为观察组和对照组,每组45例。观察组采用腹腔镜手术治疗,对照组采用开腹手术治疗,比较2组患者的近期疗效、手术情况、术后疼痛评分、术后并发症发生率及术后恢复情况。结果显示,观察组和对照组的客观缓解率分别为80.00%和75.56%,组间比较差异无统计学意义(P>0.05);观察组手术时间和术中出血量短于或少于对照组(P <0.05),术后疼痛评分及术后并发症发生率均低于对照组(P <0.05);观察组术后肛门排气恢复时间、离床活动时间及住院时间均短于对照组(P <0.05)。结果表明,腹腔镜手术、开腹手术用于结直肠癌患者治疗中均具有良好的临床疗效,但腹腔镜手术具有微创性,可减轻患者手术创伤,具有术后疼痛轻、并发症少、术后恢复快等优势。To observe the clinical efficacy of laporoscopic or laparotomy surgery for colorectal carcinoma (CC),90cases CC to be subject to surgery were randomly divided into observation group and control group each group 45cases:the former received laparoscopic surgery,the latter received laparotomy surgery;then both group's short-term effect,status on operation,the score on postoperative pain,postoperative complication, and patient's recovered status after surgery were compared.As results,in objective relief rate between the former and the latter there was no statistical difference(80.00%vs 75.56%,P >0.05);in operative time and intraoperative bleeding volume the former was significantly and respectively shorter and less than the latter(all,P <0.05);in the score on postoperative pain and complication incidence the former was significantly lower than the latter(all,P <0.05);after surgery in the time from anal exhaust recovery,for ambulation,and for hospitalization the former was all significantly shorter than control group(all,P < 0.05).Results show that above mentioned procedures for colorectal carcinoma all have better effect,but laparoscopic surgery has such advantages as minimally invasive so that reducing trauma from operation,hence slight postoperative pain,less complication and fast postoperative recovery,etc.
