摘　　要：目的 :分析尿核基质蛋白22(NMP22)检测在诊断膀胱癌中的应用价值。方法 :选取2015年12月至2018年2月期间在绵竹市人民医院就诊的108例疑似膀胱癌患者为研究对象。对这108例患者均进行膀胱镜活检(将该检查结果作为金标准)、尿脱落细胞学检查和NMP22检测。对比用NMP22检测和尿脱落细胞学检查诊断膀胱癌的敏感度与特异度,比较用NMP22检测诊断不同病理分级和不同病理分期膀胱癌的阳性率。结果 :与对这108例患者进行尿脱落细胞学检查相比,在对其进行NMP22检测时诊断其膀胱癌的敏感度较高,P <0.05。对这108例患者进行尿脱落细胞学检查和NMP22检测的特异度相比,P>0.05。进行NMP22检测诊断G1～G2级膀胱癌的阳性率低于诊断G3级膀胱癌的阳性率,P <0.05。进行NMP22检测诊断pTa～T1期膀胱癌的阳性率低于诊断pT2～T4期膀胱癌的阳性率,P <0.05。结论 :进行NMP22检测诊断膀胱癌的敏感度较高,且可快速准确地判断膀胱癌的病理分级和病理分期。objective: to analyze the application value of NMP22 in the diagnosis of bladder cancer. Methods: 108 patients with suspected bladder cancer admitted to mianzhu people’s hospital from December 2015 to February 2018 were selected as the study subjects. Cystoscopic biopsies(the gold standard), urine shed cytology, and NMP22 were performed in all 108 patients. The sensitivity and specificity of NMP22 detection and urine exfoliation cytology in the diagnosis of bladder cancer were compared,and the positive rate of NMP22 detection in the diagnosis of bladder cancer with different pathological grades and different pathological stages was compared. Results: the sensitivity of NMP22 in the diagnosis of bladder cancer was higher than that of the 108 patients who underwent urine ablative cytology, P < 0.05. The specificity of urine abscess cytology and NMP22 detection in these 108 patients was P > 0.05. The positive rate of grade g1-g2 bladder cancer diagnosed by NMP22 was lower than that of grade G3 bladder cancer, P < 0.05. The positive rate of pt2-t4 bladder cancer was lower than that of pt2-t4 bladder cancer detected by NMP22, P < 0.05. Conclusion: the sensitivity of NMP22 in the diagnosis of bladder cancer is high, and it can quickly and accurately determine the pathological grade and stage of bladder cancer.
